This role ensures that services are delivered in accordance with applicable federal, state, and local standards, guidelines, and regulations, maintaining the highest degree of quality care for our patients.</p>\r\n\r\n<p><strong>Key Responsibilities:</strong></p>\r\n\r\n<ul>\r\n\t<li>Collaborate with administration, medical and nursing staff, patients, and families in planning comprehensive therapy services.</li>\r\n\t<li>Assist in the development, implementation, and coordination of individualized patient care plans, ensuring alignment with patient assessments.</li>\r\n\t<li>Clearly brief patients on procedures involved in occupational therapy and prepare necessary equipment, providing reassurance before and during treatment.</li>\r\n\t<li>Determine proper equipment usage, application, and body positioning, making necessary adjustments during therapy treatments to optimize patient outcomes.</li>\r\n\t<li>Recommend modifications or changes to the patient&#39;s therapy program based on ongoing evaluation of their progress.</li>\r\n\t<li>Ensure all therapy notes are informative, descriptive, and adhere to the Company&rsquo;s policy and procedures.</li>\r\n</ul>\r\n\r\n<p><strong>Requirements:</strong></p>\r\n\r\n<ul>\r\n\t<li>Minimum of one (1) year of experience in a skilled nursing facility setting.</li>\r\n\t<li>Associate&rsquo;s Degree in Occupational Therapy from an accredited program.</li>\r\n\t<li>Current Occupational Therapy Assistant (COTA) license or eligibility for licensure in the state of Colorado.</li>\r\n</ul>\r\n\r\n<p><strong>Medical Specialties:</strong></p>\r\n\r\n<ul>\r\n\t<li>Geriatrics</li>\r\n\t<li>Physical &amp; Rehabilitation Medicine</li>\r\n</ul>\r\n\r\n<p><strong>Why Join The Center at Cordera?</strong> Be part of a compassionate and dedicated team committed to making a tangible difference in our patients&#39; lives.
