Description

The Bone Marrow Transplant Survivorship Advanced Practice Provider provides comprehensive, longitudinal care to pediatric and adult patients following hematopoietic stem cell transplantation, with a particular focus on survivors of pediatric transplant transitioning into adulthood. This role emphasizes late effects monitoring, chronic condition management, survivorship care planning, and coordination of multidisciplinary care. The APP also performs medical evaluations of healthy related stem cell donors, both pediatric and adult, in close collaboration with transplant program leadership, coordinators, and primary care providers. Physician Assistant Master’s or Doctoral degree of Physician Assistant Studies from an accredited program and eligibility for medical staff credentialing required. Clinical Nurse Specialist Master’s or Doctorate degree in the clinical specialty area in which they practice from an accredited program and eligibility for medical staff credentialing required. (Required) Minimum Work Experience 0 years Minimum of 0 years of experience as an Advanced Practice Provider (Required) Required Skills/Knowledge Strong knowledge of clinical specialty and services. Demonstrates clinical leadership skills. Excellent written and verbal communication skills. Required Licenses and Certifications Advanced Practice Registered Nurse (APRN) Nurse Practitioner 1. Licensure in the state(s) of clinical practice 2. CDS in state(s) of clinical practice 3. DEA (Required) Or Physician Assistant 1. Licensure in the state(s) of clinical practice 2. CDS in state(s) of clinical practice 3. DEA (Required) Or Advanced Practice Registered Nurse (APRN) Clinical Nurse Specialist 1. Licensure in the state(s) of clinical practice If prescribing controlled substances: 2. CDS in state(s) of clinical practice 3. DEA (Required) Or . Nurse Practitioner 1. Active national certification with governing body such as PNCB, ANCC, or AANPCB 2. BLS certification (Required) Physician Assistant 1. Active national certification with NCCPA 2. BLS certification (Required) (Required) Basic Life Support for Healthcare Provider (BLS) (Required) Functional Accountabilities Clinical Assesses, diagnoses, and monitors clinical status of each patient utilizing evidenced based practice and medical knowledge. Develops appropriate plan of care. Writes patient care orders, prescribes therapy, and provides necessary home care orders as needed. Provide comprehensive, longitudinal survivorship care to pediatric and adult post‑transplant patients, including management of late effects, chronic conditions, and transition from pediatric to adult care. Maintain a dedicated continuity survivorship clinic, serving as the primary clinical point of contact and coordinating multidisciplinary care using evidence‑based survivorship guidelines. Develop and implement individualized survivorship care plans, deliver patient and family education, and address psychosocial, developmental, and self‑management needs across the lifespan. Perform medical evaluations and clearance for healthy related stem cell donors and partner with program leadership on survivorship program development and quality improvement initiatives. Identifies significant changes in patient status using physical exam, lab values, diagnostic tests, and patient/family input. Differentiates between normal and abnormal findings. Performs procedures as needed after obtaining appropriate credentialing. Participates in and/ or leads multidisciplinary patient care. Is a clinical resource for patient, parents and the interdisciplinary team. Appropriately communicates and escalates clinical/administrative issues. Collaborates with nursing, medicine, and other members of the healthcare team to regularly ensure quality patient care. Administrative Serves as a clinical preceptor to NP, CNS or PA students. Acts as a clinical resource for staff in patient care issues. Provides formal/informal educational presentation to staff on areas of expertise. Participates and leads quality improvement initiatives in their department.
