You’ll deliver community supports (e.g., housing deposits, navigation, tenancy sustainment, day habilitation/skills-building) and partner with landlords to secure and stabilize permanent housing for homeless families.</p>\n<p><br></p>\n<p><span style=\"font-family: Arial, sans-serif; font-size: 12pt; font-weight: bold\">What you’ll do</span></p>\n<ul>\n<li><span style=\"font-family: Arial, sans-serif; font-size: 12pt\">Complete comprehensive 5x5 assessments and create Family Service Plans.</span></li>\n<li><span style=\"color: #242424; font-family: Arial,sans-serif; font-size: 12pt\">Provide field-based services, including travel to multiple service locations.</span></li>\n<li><span style=\"font-family: Arial, sans-serif; font-size: 12pt\">Provide trauma-informed case management: referrals, coaching, goal tracking, and wellness checks.</span></li>\n<li><span style=\"font-family: Arial, sans-serif; font-size: 12pt\">Lead the housing process end-to-end: search, applications, landlord communication/negotiation, lease-up, and advocacy.</span></li>\n<li><span style=\"font-family: Arial, sans-serif; font-size: 12pt\">Document services in HMIS, EHR, and internal systems; maintain accurate case files.</span></li>\n<li><span style=\"font-family: Arial, sans-serif; font-size: 12pt\">Conduct habitability/health &amp; safety checks on units.</span></li>\n<li><span style=\"font-family: Arial, sans-serif; font-size: 12pt\">Mediate landlord/tenant issues (late rent, repairs, neighbor concerns) with the housing stabilization team.</span></li>\n<li><span style=\"font-family: Arial, sans-serif; font-size: 12pt\">Collaborate with partner agencies; attend case reviews, community meetings, and required CalAIM trainings.</span></li>\n</ul>\n<p><span style=\"font-family: Arial, sans-serif; font-size: 12pt\"><br><br></span></p>\n<p><span style=\"font-family: Arial, sans-serif; font-weight: bold\">and</span><span style=\"font-family: Arial, sans-serif\"> 2+ years in human services (or equivalent relevant experience).</span></span></li>\n<li><span style=\"font-family: Arial, sans-serif; font-size: 12pt\">Experience with people experiencing homelessness; families preferred.</span></li>\n<li><span style=\"font-family: Arial, sans-serif; font-size: 12pt\">Working knowledge of fair housing laws and landlord/tenant practices.</span></li>\n<li><span style=\"font-family: Arial, sans-serif; font-size: 12pt\">Excellent communication, problem-solving, and boundary-setting; calm under pressure.</span></li>\n<li><span style=\"font-family: Arial, sans-serif; font-size: 12pt\">Commitment to trauma-informed, culturally responsive care.</span></li>\n<li><span style=\"font-family: Arial, sans-serif;
