Either on  a Full Time Basis or consistent PRN basis! </p>\n<p><br></p>\n<p><span style=\"font-weight: bold\">Overview</span><br><br></p>\n<p>We provide patients and their families a complete range of home care services,</p>\n<p> </p>\n<p>Specialty home care programs also are available, including adult cardiac, maintenance therapy, orthopedic rehabilitation and much more. </p>\n<p> </p>\n<p><span style=\"font-weight: bold\">Role Purpose</span></p>\n<p>Provides direct patient care activities to home care or home health patients according to physician orders in the home environment including assessment, evaluation, treatment planning, implementation and instruction/supervision of physical therapy practice.</p>\n<p><br></p>\n<p><span style=\"font-weight: bold\">Responsibilities</span></p>\n<ul>\n<li>Instructs and assists patient to perform various physical activities and in use of assistive and supportive devices such as crutches, canes, and prostheses.</li>\n<li>Instructs patient and family in treatment procedures.</li>\n<li>Re-evaluates patient and adapts treatment program accordingly to achieve maximum benefit.</li>\n<li>Confers with physician and other practitioners to obtain additional information, suggest revisions in treatment plan and to integrate physical therapy treatment with other aspects of patients? </li>\n</ul>\n<p> </p>\n<p> </p>\n<ul>\n<li><span style=\"font-weight: bold\">Licenses &amp; Certifications</span></li>\n<li>Valid Driver's License</li>\n<li>State License</li>\n<li><br><br><span style=\"font-weight: bold\">To learn more,  please complete an application. </span></li>\n<li>\n</li></ul>",
