Nurses must be willing and able to travel throughout the service area based on patient needs.</span> <br><br><span style=\"color: rgb(0, 0, 0)\">You will work directly with specialty pharmacies and Certified Home Health Agencies (CHHAs) to deliver critical clinical care to patients transitioning from hospital to home.</span> <br><span style=\"color: rgb(0, 0, 0)\"><br></span></p>\n<p><span style=\"color: rgb(0, 0, 0)\"><br></span></p>\n<p><span style=\"color: rgb(0, 0, 0)\"><span style=\"font-weight: bold\">Compensation:</span> $55-70/hr</span></p>\n<p><span style=\"color: rgb(0, 0, 0)\"><span style=\"font-weight: bold\">Reporting to:</span> Senior Director of Patient Services</span> <span style=\"color: rgb(0, 0, 0)\"><br></span><br><span style=\"color: rgb(0, 0, 0)\"><span style=\"font-weight: bold\">Key Responsibilities</span></span></p>\n<ul>\n<li><span style=\"color: rgb(0, 0, 0)\">Perform start of care visits and follow-up visits as directed by physician orders</span></li>\n<li><span style=\"color: rgb(0, 0, 0)\">Serve as liaison between pharmacy, CHHA, and care team to coordinate all aspects of patient care</span></li>\n<li><span style=\"color: rgb(0, 0, 0)\">Provide skilled nursing care for infusion therapy in the home setting</span></li>\n<li><span style=\"color: rgb(0, 0, 0)\">Manage and care for central venous catheters (CVC), including PICC lines, Mediports, Hickman, and Broviac</span></li>\n<li><span style=\"color: rgb(0, 0, 0)\">Educate patients and families on infusion therapy and CVC management</span></li>\n<li><span style=\"color: rgb(0, 0, 0)\">Conduct ongoing patient assessments, including dressing changes, blood draws, and care management</span></li>\n<li><span style=\"color: rgb(0, 0, 0)\">Communicate changes in patient condition to the physician promptly</span></li>\n<li><span style=\"color: rgb(0, 0, 0)\">Maintain accurate and timely clinical documentation in accordance with agency policies</span></li>\n<li><span style=\"color: rgb(0, 0, 0)\">Ensure strict adherence to HIPAA and patient confidentiality standards</span></li>\n</ul>\n<p><span style=\"color: rgb(0, 0, 0)\"><br></span><br><span style=\"color: rgb(0, 0, 0)\"><span style=\"font-weight: bold\">Cases May Include</span></span></p>\n<ul>\n<li><span style=\"color: rgb(0, 0, 0)\">Pediatrics and Adult patients</span></li>\n<li><span style=\"color: rgb(0, 0, 0)\">Start of Care visits (often in the evening post-discharge)</span></li>\n<li><span style=\"color: rgb(0, 0, 0)\">IVIG/IGG (Immunoglobulin)</span></li>\n<li><span style=\"color: rgb(0, 0, 0)\">Antibiotic therapy</span></li>\n<li><span style=\"color: rgb(0, 0, 0)\">Hydration therapy</span></li>\n<li><span style=\"color: rgb(0, 0, 0)\">TPN</span></li>\n<li><span style=\"color: rgb(0, 0, 0)\">Blood draws</span></li>\n<li><span style=\"color: rgb(0, 0, 0)\">PICC line dressing changes and removal</span></li>\n<li><span style=\"color: rgb(0, 0, 0)\">Chemotherapy</span></li>\n<li><span style=\"color: rgb(0, 0, 0)\">Simple wound care</span></li>\n<li><span style=\"color: rgb(0, 0, 0)\">Specialty midline insertion</span></li>\n<li><span style=\"color: rgb(0, 0, 0)\">Fertility injections (Makena)</span></li>\n</ul>\n<p><span style=\"color: rgb(0, 0, 0)\"><br></span><br><span style=\"color: rgb(0, 0, 0)\"><span style=\"font-weight: bold\">Qualifications:</span></span></p>\n<ul>\n<li><span style=\"color: rgb(0, 0, 0)\">Applicants must have at least 3 years  of experience as a registered nurse in a home care infusion company, hospital, clinic, or medical facility.</span></li>\n<li><span style=\"color: rgb(0, 0, 0)\">Infusion experience is REQUIRED.</span></li>\n<li><span style=\"color: rgb(0, 0, 0)\">Fluency in Spanish a huge plus.</span></li>\n<li><span style=\"color: rgb(0, 0, 0)\">Knowledge of principles &amp;
