This position focuses on performing thorough clinical assessments, diagnosing and managing acute and chronic conditions, and developing individualized treatment plans to promote optimal health outcomes. The practitioner collaborates closely with physicians, specialists, and other healthcare professionals to ensure coordinated and effective patient care. Additionally, the role involves educating patients and their families on health maintenance, disease prevention, and medication management while adhering to all regulatory and safety standards. Ultimately, this position aims to enhance access to quality healthcare within the community by providing advanced clinical expertise and compassionate care.</p><p><strong>Minimum Qualifications:</strong></p><ul><li>Current and unrestricted Nurse Practitioner (NP) or Physician Assistant (PA) license in the practicing state.</li><li>Certification in Basic Life Support (BLS) from an accredited organization.</li><li>Comprehensive knowledge of DEA regulations related to prescribing controlled substances and active registration.</li><li>Demonstrated experience in advanced clinical practice within a family practice or primary care setting.</li><li>Strong clinical assessment and diagnostic skills with the ability to manage a wide range of patient conditions.</li><li>At least one of year of NP or PA practice.</li></ul><p><strong>Preferred Qualifications:</strong></p><ul><li>Experience with electronic health record (EHR) systems and telehealth platforms.</li><li>Additional training or certification in chronic disease management or geriatric care.</li><li>Previous work experience in a multidisciplinary healthcare environment.</li><li>Participation in continuing education programs related to family practice or primary care.</li><li>Two years in primary care.</li></ul><p><strong>Responsibilities:</strong></p><ul><li>Conduct detailed patient histories and physical examinations to assess health status and identify medical conditions.</li><li>Order, interpret, and follow up on diagnostic tests and laboratory results to inform clinical decision-making.</li><li>Develop and implement evidence-based treatment plans, including prescribing medications in compliance with DEA regulations.</li><li>Provide patient education on disease prevention, lifestyle modifications, and medication adherence to support long-term wellness.</li><li>Collaborate with interdisciplinary healthcare teams to coordinate patient care and facilitate referrals to specialists as needed.</li><li>Maintain accurate and timely documentation of patient encounters in electronic health records.</li><li>Respond promptly to patient inquiries and manage urgent clinical situations using advanced clinical skills and Basic Life Support techniques.</li></ul><p><strong>Skills:</strong></p><p>The required skills such as clinical assessment and advanced clinical practice are essential for accurately evaluating patient conditions and formulating effective treatment plans on a daily basis.
