Job Summary:
Provide professional physical therapy to patients in an in-home setting or in our Physical Therapy Department. Assists in the rehabilitation and restoration of normal body functions following illness or injury.
Requires supervision of a licensed Physical Therapist.
Essential Functions:
Performs planned services that are delegated and supervised by the Physical Therapist to meet the patient’s goals and needs. Help patient accomplish treatment plan and accept therapeutic devices by administering manual exercises, instructing, encouraging and assisting patients in performance of physical activities, such as ambulatory function, activities and daily living activities and use of support devices as needed. Follow the Plan of Care while adhering to Agency policy under the supervision of the Physical Therapist. Meets regularly with the PT to update client progress and needs. Maintains patient rights and dignity. Completes and submits documentation timely and accurately according to Agency policy.
Additional Responsibilities:
Performs other duties as assigned or requested Conforms to all applicable Serve Link Home Care, Inc. policies and procedures Participates actively in continuing education and in-services Maintains confidentiality of patient information and business trade practices Assumes accountability for reporting incidents and complaints according to Agency policy
Knowledge / Skills / Abilities:
Ability to read and interpret documents such as safety rules, operating and maintenance instructions, and procedure manuals. Ability to write routine reports and correspondence. Ability to speak effectively before groups of clients or employees of the organization Ability to compute rate, ratio, and percent. Ability to add, subtract, multiply, and divide in all units of measure, using whole numbers, common fractions, and decimals. Ability to solve practical problems ad deal with a variety of concrete variables in situations where only limited standardization exists. Ability to interpret a variety of instructions furnished in written, oral, diagram, or schedule form.
Age-Related Competencies:
Demonstrates the basic knowledge and skills necessary to identify age-specific patient needs appropriate for this position
Risk Management:
Cooperates fully in all risk management activities and investigations Keeps abreast of changes in health care law Maintains Agency/program compliance with local, state and federal laws as well as state accreditation standards
Minimum Position Qualifications:
Education: PTA graduate of school accredited by Commission on Accreditation for Physical Therapy Training: Appropriate competency assessments completed License/Certification: Current licensure from applicable State Board of Physical Therapy Examiners; CPR certification; Current driver’s license and proof of auto liability insurance
Preferred Position Qualifications:
Experience: 1 year in a rehab setting
