Anchored at RFHN’s East Palo Alto location, the SIT extends medical home services into school settings in a primarily virtual manner while maintaining a consistent on-site presence to reduce barriers to care and address health barriers to learning.</span></span></p>\n\n<p><br />\n<span style=\"font-size:14px;\"><span style=\"font-family:Arial, Helvetica, sans-serif;\">Reporting to the Associate Medical Director for Pediatrics and the Pediatrics Operations Manager, the Team Lead is responsible for planning, launching, and scaling pilot school partnerships over a four-year grant period. This includes overseeing development of standardized, bidirectional release-of-information processes between Ravenswood City School District and medical homes (in alignment with HIPAA, FERPA, and other applicable statutes), coordinating preventive health screenings and integrated services, strengthening care coordination and family engagement, and using data to demonstrate improvements in
