The Mental Health Services Provider and client will work together to identify goals and create a treatment plan to work towards achieving those goals through office-based counseling/mental health services. The Mental Health Services Provider will utilize various assessment tools to help gauge client progress on reaching their goals. The goal of mental health services/therapy is to increase the daily life functioning of the clients they are serving. The Mental Health Services Provider may provide individual, family therapy, and/or group sessions, depending on the needs of the individual receiving services.</span></p>\n<p><br></p>\n<p><span style=\"color: #f47c48; font-family: Lato, sans-serif; font-size: 12pt; font-weight: bold; text-decoration: none\">ESSENTIAL DUTIES AND RESPONSIBILITIES</span></p>\n<p><br></p>\n<ul>\n<li><span style=\"font-size: 16px\">Provide structured, goal-orientated therapy for individuals and/or families affected by or at risk of physical, sexual, emotional, substance use abuse as well as neglect, mental health issues, and/or behavioral issues.</span></li>\n<li><span style=\"font-size: 16px\">Provide therapy to clients in an individual, group, and/or family setting.</span></li>\n<li><span style=\"font-size: 16px\">Complete initial, ongoing, and final assessments to evaluate client/family needs and progress.</span></li>\n<li><span style=\"font-size: 16px\">Complete initial treatment plan within 7 days of initial face to face contact, review ongoing with client/family every 30 days, and be re-evaluated/updated ongoing every 90 days.</span></li>\n<li><span style=\"font-size: 16px\">Attend, communicate, and correspond with Child and Family Team Meetings (CFTM) as needed.</span></li>\n<li><span style=\"font-size: 16px\">Provide services based on objectives originating from the DCS/Probation case plan/Informal Adjustment.</span></li>\n<li><span style=\"font-size: 16px\">Attend court hearings and/or appeals when testimony is requested.</span></li>\n<li><span style=\"font-size: 16px\">Schedule provision of services at a time convenient for the individual/family.</span></li>\n<li><span style=\"font-size: 16px\">Completion of daily progress note and a monthly report for each client no later than the 5th day of the month at midnight for the previous month.</span></li>\n<li><span style=\"font-size: 16px\">Upload and email written reports monthly utilizing DCS Monthly Progress Report template no later than the 10th of each month.</span></li>\n<li><span style=\"font-size: 16px\">Ensure services provided reflect goals and outcomes as outlined in the DCS Standard of Counseling.</span></li>\n<li><span style=\"font-size: 16px\">Actively participate in supervision/consultation at least 1 hour per 20 hours of direct client services provided, but not less than once every 2 weeks.</span></li>\n<li><span style=\"font-size: 16px\">Participate in on-call rotation to be available to clients/families 24/7 via phone, or in person, for crisis intervention.</span></li>\n<li><span style=\"font-size: 16px\">Obtain all needed signatures for required paperwork including intake/annual documents.</span></li>\n<li><span style=\"font-size: 16px\">Responsible for bringing paperwork to the office for the purpose of filing copies in the main file. </span></li>\n<li><span style=\"font-size: 16px\">Responsible for making sure clients’ files are current and up-to-date both in the main file and the electronic file.</span></li>\n<li><span style=\"font-size: 16px\">Ensure all billing/payroll units and hours are accurate prior to submission for billing/payroll.</span></li>\n<li><span style=\"font-size: 16px\">Consistently provide therapeutic services and follow up with service providers and families.</span></li>\n<li><span style=\"font-size: 16px\">Exhibit effective correspondence and communication with all providers, family members, staff members, clients and all other treatment team members as applicable. Be sure to return phone calls, text messages, and emails within a 24-hour time frame.</span></li>\n<li><span style=\"font-size: 16px\">Carry and maintain current professional liability insurance, as applicable.</span></li>\n<li><span style=\"font-size: 16px\">Adhere to professional and ethical interaction policies and procedures.</span></li>\n<li><span style=\"font-size: 16px\">Position requires travel to team meetings and court hearings, must be willing to use personal vehicle and provide proof of current automobile insurance and registration.</span></li>\n<li><span style=\"font-size: 16px\">Position consists of growing a caseload as referrals are received.</span><br></li>\n</ul>\n<p><br></p>\n<p><span style=\"color: #f47c48; font-family: Lato, sans-serif; font-size: 12pt; font-weight: bold; text-decoration: none\">ORGANIZATIONAL RELATIONSHIPS</span></p>\n<p><br></p>\n<p><span style=\"color: #000000; font-family: Lato, sans-serif; font-size: 12pt; text-decoration: none\">Must be able to work well and communicate with clients, family members, providers, supervisor, and all other treatment team members as applicable.</span></p>\n<p><br></p>\n<p><span style=\"color: #f47c48; font-family: Lato, sans-serif; font-size: 12pt; font-weight: bold; text-decoration: none\">QUALIFICATIONS AND EDUCATION</span></p>\n<p><br></p>\n<ul>\n<li><span style=\"font-size: 16px\">Must be a licensed provider by means of one of the following:</span>\n<ul>\n<li><span style=\"font-size: 16px\">A Master’s or Doctorate degree with a current license issued by the Indiana Behavioral Health and Human Services Licensing Board</span></li>\n<li><span style=\"font-size: 16px\">Master’s degree with temporary permit issued by the Indiana Behavioral Health and Human Services Licensing Board</span></li>\n<li><span style=\"font-size: 16px\">Hold a Master’s or Doctorate degree that is applicable for licensure. Regular use of a telephone, email, or other medium to communicate is essential.</span></li>\n<li><span style=\"font-size: 16px\">Sitting for extended periods is common.  </span></li>\n<li><span style=\"font-size: 16px\">Hearing and vision within normal ranges is essential for normal conversations, to receive ordinary information, and to prepare or inspect documents.</span></li>\n<li><span style=\"font-size: 16px\">No heavy lifting is expected.
