(TTC) that may be appropriate for the patient&rsquo;s family members.</li><li>Attends in-services trainings and educational workshops.</li><li>Attends all required staff meetings.</li><li>Cooperates and communicates fully with other staff members regarding patient behavior, progress activities, etc.</li><li>As appropriate works closely with other multi-disciplinary team members to develop a comprehensive Individualized Treatment Plan on assigned patients and to ensure that all identified problems are addressed.</li><li>Assist with employment/vocational referrals, provide vocational counseling re resume preparation, job interviewing etc.</li><li>Provides outreach and follow up services to patients in and out of treatment, to check progress, offer additional services, help reengage in treatment, make needed referrals.</li><li>Facilitates discharge-planning efforts, commencing at time of admission on all assigned patients, including areas of housing, employment, medical psychiatric, financial, aftercare, etc.</li><li>Makes direct contact within agency and with outside entities as appropriate, in order to assist patients in resolution of legal, financial, housing, medical, psychiatric and discharge planning issues</li><li>Provides regular, comprehensive progress note charting in patient&rsquo;s Medical Record which document contacts, progress towards identified goals, program participation, referrals, discharge planning and other case management activities.</li><li>Performs other duties as assigned by Director and/or Administrator.</li></ol><p><strong>EXPERIENCE</strong></p><p>1. &nbsp; &nbsp; &nbsp; &nbsp; For AOD Programs:</p><ul><li>Enrollment in an academic Addiction Studies program and registration with CCAPP, CAADE, or CADTP&nbsp;</li><li>1 year experience in a Behavioral Healthcare setting.</li></ul><p>2. &nbsp; &nbsp; &nbsp; &nbsp; For all other Programs:</p><ul><li>1 year experience in a Behavioral Healthcare setting (HIV programs) and DVSS Certification for DV specific staff</li></ul><p>3. &nbsp; &nbsp; &nbsp; &nbsp; Bilingual English/Spanish preferred</p><p>4. &nbsp; &nbsp; &nbsp; &nbsp; Valid driver&rsquo;s license and insurance</p><p><strong>LICENSES/CERTIFICATES</strong></p><p>For AOD programs, registration or certification with a DHCS approved state level counselor certification agencies.</p><p><strong>SETTING</strong></p><p>Tarzana Treatment Centers, Inc. is a non-profit corporation, which provides behavioral healthcare services, including chemical dependency.</p><p><strong>EQUAL OPPORTUNITY EMPLOYER</strong></p><p>Tarzana Treatment Centers, Inc. does not discriminate as to race, nationality, religion, gender, sexual orientation and disability in its hiring practices.</p><p><strong>ADA REQUIREMENT</strong></p><p>Tarzana Treatment Centers, Inc. supports the Americans with Disabilities Act.
