Description
For 50 years, Buckelew Programs has been committed to the mission of providing mental health and addiction services that promote recovery, resilience and hope. The agency provides numerous essential community services throughout Marin, Sonoma and Napa counties ranging from supported housing and job training for people with mental illness to outpatient psychotherapy and other support for children, adults and families, as well as residential detoxification services, and a regional suicide prevention program and crisis hotline.
GENERAL SUMMARY:
The Registered Nurse (RN) is a new position to the agency and will provide nursing assessments, diabetes screening and education for clients in multiple programs. In this role, you will implement plans of care, advise clients on health maintenance and prevention, maintain clinical documentation, mentor other staff, and/or provide case management.
RESPONSIBILITIES AND DUTIES:
Client Care & Education
Performs client care activities that are assigned within the scope of competencies Collaborates with the Care team and Providers to increase understanding, involvement and to enhance client experience Performs nurse visits under the direction of Provider (s) (includes, but not limited to): wound care; B12; Coumadin; Testosterone injections; Insulin and glucometer teaching; Depo; BP checks; suture removal; medication reconciliation; and patient education for chronic disease per protocol Assists with various procedures Informs and educates patients on the significance of abnormal lab results relative to chronic disease and/or Public Health tracked illnesses in accordance with agency approved educational materials as per protocol Documentation
Ensures a clean, orderly and functional work environment Assists team manager to facilitate policy implementation and adherence by staff Effectively participates in problem identification and resolution Promotes continuous improvement of workplace safety and environmental practices Performs miscellaneous job-related duties as assigned Case Management
Performs indirect case management (includes, but not limited to): patient recalls; missed breast care follow up; and new patient intakes Provides direct case management for uncontrolled chronic disease with Provider direction Training
Assists and leads staff trainings Provides guidance to MA’s doing non-appointment work Mentors and orients new staff, students and other team members
QUALIFICATIONS:
Education/Experience
Two year degree in Nursing required; Bachelors and/or Master’s degree is preferred Equivalent to an RN or higher degree from an accredited school of nursing One to two years of increasingly responsible nursing experience including experience in community nursing preferred Skills
