Description

For 50 years, Buckelew Programs has been committed to the mission of providing mental health and addiction services that promote recovery, resilience and hope. The agency provides numerous essential community services throughout Marin, Sonoma and Napa counties ranging from supported housing and job training for people with mental illness to outpatient psychotherapy and other support for children, adults and families, as well as residential detoxification services, and a regional suicide prevention program and crisis hotline. GENERAL SUMMARY: The Registered Nurse (RN) is a new position to the agency and will provide nursing assessments, diabetes screening and education for clients in multiple programs. In this role, you will implement plans of care, advise clients on health maintenance and prevention, maintain clinical documentation, mentor other staff, and/or provide case management. RESPONSIBILITIES AND DUTIES: Client Care & Education Performs client care activities that are assigned within the scope of competencies Collaborates with the Care team and Providers to increase understanding, involvement and to enhance client experience Performs nurse visits under the direction of Provider (s) (includes, but not limited to): wound care; B12; Coumadin; Testosterone injections; Insulin and glucometer teaching; Depo; BP checks; suture removal; medication reconciliation; and patient education for chronic disease per protocol Assists with various procedures Informs and educates patients on the significance of abnormal lab results relative to chronic disease and/or Public Health tracked illnesses in accordance with agency approved educational materials as per protocol Documentation Ensures a clean, orderly and functional work environment Assists team manager to facilitate policy implementation and adherence by staff Effectively participates in problem identification and resolution Promotes continuous improvement of workplace safety and environmental practices Performs miscellaneous job-related duties as assigned Case Management Performs indirect case management (includes, but not limited to): patient recalls; missed breast care follow up; and new patient intakes Provides direct case management for uncontrolled chronic disease with Provider direction Training Assists and leads staff trainings Provides guidance to MA’s doing non-appointment work Mentors and orients new staff, students and other team members QUALIFICATIONS: Education/Experience Two year degree in Nursing required; Bachelors and/or Master’s degree is preferred Equivalent to an RN or higher degree from an accredited school of nursing One to two years of increasingly responsible nursing experience including experience in community nursing preferred Skills An energetic and creative individual with high standards and an appropriate professional image Proven ability to handle multiple projects and meet deadlines Versatility, flexibility and willingness to work within an ambiguous, fast-paced environment Demonstrated resourcefulness in providing solutions and setting priorities Effective interpersonal skills to work effectively with all levels within the health center Strong verbal and written communication skills with demonstrated ability to communicate clearly and effectively at all levels Knowledge of Principles, procedures, methods and techniques of nursing and health care Modern practices and principles of method disease counseling A variety of community resources Basic management practices and principles Principles and procedures of electronic medical record keeping Universal precautions and infection control practices Pertinent Federal, State and local laws, codes, and regulations regarding health care Ability to Provide administrative and nursing orientation to staff Assess and evaluate patient responses to service provided, treatment, and medication Demonstrated leadership skills and a desire to continually enhance nursing practice and performance Prepare clear and concise documentation and reports Respond to requests and inquiries from the general public Apply basic time management and patient care coordination skills to effectively provide nursing services Establish and maintain effective working relationships with those contacted in the course of work, including service representatives and the general public Plan and organize multiple work tasks, problem-solve and analyze information toward sound decisions Identify problems/issues, develop and evaluate options in order to implement solutions Appropriately manage highly sensitive and confidential patient information Ability to effectively present information and respond to questions and requests from patients, co-workers, and others as necessary. Bilingual in English and Spanish, both written and verbal preferred. Licenses and Certifications Possession of an appropriate, valid California Registered Nursing License Possession of, or ability to obtain, an appropriate valid California driver’s license Possession of, or ability to obtain, an appropriate valid CPR certificate Possession of, or ability to obtain, an appropriate valid BLS certificate Physical Requirements: The physical demands described here are representative of those that must be met by an employee to successfully perform the essential functions of this job.
