Overview
Manager of Nursing (Field) Overview
The Manager of Nursing will ensure effective and efficient completion of tasks associated with the clinical assessment process including but not limited to clinical onboarding, UAS-NY and NYIA assessment review, risk score review, and post-hospital discharge evaluation. The Manager of Nursing serves as a valuable resource to the Care Management team by documenting critical clinical caseloads, reassessments, and change in condition information in the electronic record system. The Manager of Nursing partners with the Social Work team by coordinating services and community resources and meeting member socioeconomic needs to support the quality of life.
Responsibilities
Manager of Nursing (Field) Essential Job Functions
Manages a team of clinical assessment nurses and reviewers to ensure the seamless and timely completion and quality of assessments while also focusing on quality, gaps in care, and utilization.
Assist the Director of Nursing in planning, developing, organizing, implementing, evaluating, and directing the day-to-day functions of the RN Assessment Team.
Responsible for staff performance evaluations, administering discipline, training and/or coaching.
Makes written and oral reports/recommendations to the Director of Nursing as necessary/required, concerning the operation of the RN Assessment Team.
Responsible for UAS-NY and NYIAP oversight for Elderwood IPA.
Participate in the utilization review process and evaluate to determine if the member’s condition and needs meet criteria for covered services.
Delivers member specific metrics and coaches RNs on gap closing opportunities.
Collaborates with RN Assessment Team to improve performance in areas of Quality, Risk Adjustment and Operations.
Compare assessment data to baseline assessment to monitor Members’ progress, recognize alterations in function, including life-threatening situations, and intervene and document appropriately.
Serves as clinical point person in response to Care Delivery questions, concerns, or issues
Ensure that documentation in the care management record meets all applicable professional standards, using an EMR for each observation, verbal report, or interaction with the Member, Member’s caregiver/family, PCP or other provider, whether by home visit, telephonic, or written interaction.
Participate in Disease Management, Utilization Management (as required), and Quality Improvement activities.
Utilize electronic timekeeping system as directed.
Arrive to work on time, regularly, and works as scheduled.
Recognize and follow the dress code of the facility, including wearing name tag at all times.
Follow policy and procedure regarding all electronic devices, computers, tablets, etc.
Support and abide by Elderwood IPA’s Mission, Vision, and Values.
Abide by Elderwood IPA’s businesses code of conduct, compliance and HIPAA policies.
Performs other duties as assigned by supervisor, management staff, or Administrator.

Qualifications
Manager of Nursing (Field) Educational Requirements and Qualifications
Current New York State Registered Nurse License and a valid NYS Driver’s license in good standing required
Masters in relevant field preferred, or BSN with extensive experience accepted
Minimum of four (4) years nursing experience in home care, case management, discharge planning or managed care required
Minimum of one (1) years management experience required
Minimum of one (1) year experience working with a frail or elderly population required
Minimum of one (1) year experience with health assessments required
Bilingual skills are preferred
Manager of Nursing (Field) Skills/Competencies
Understanding of how assessments and coding impact reimbursement and ability to navigate related challenges
Must be able to perform data analysis, report on outcomes, and provide updates routinely relative to clinical activities and risk scores
Must be competent in using the UAS-NY assessment tool. Previous UAS-NY is desired, but not required. Training is available.
Previous experience with use of standard patient assessment instruments such as PRI, UAS-NY is desired, but not required.
Must have current knowledge of comprehensive case management, chronic care and geriatric issues, and best practices.
Must possess proficient computer skills, including working knowledge of Microsoft Office Suite, e-mail systems, electronic medical records, and web-based programs.

This position requires regular interaction with residents, coworkers, visitors, and/or supervisors.
