In this role, you will be responsible for providing individualized rehabilitative care to our patients in a home health setting. Your expertise will help patients regain independence, reduce pain, and improve mobility, ultimately enhancing their quality of life.</p>\n<p><strong>Key Responsibilities</strong>• <strong>Patient Assessments:</strong> Conduct thorough evaluations of patients’ physical abilities, medical history, and limitations to create personalized treatment plans.• <strong>Treatment Plans:</strong> Develop individualized rehab programs based on assessments and patients’ goals, including therapeutic exercises, manual techniques, gait training, and modalities.• <strong>Patient Care:</strong> Provide hands-on interventions to enhance mobility, strength, and flexibility, while monitoring progress and adjusting treatment as necessary.• <strong>Family & Caregiver Support:</strong> Educate patients and their families on exercises, techniques, and injury prevention strategies to continue rehabilitation outside of sessions.• <strong>Documentation:</strong> Maintain accurate and up-to-date patient records, including treatment plans, progress notes, and outcomes.• <strong>Collaboration:</strong> Work closely with physicians, nurses, and other healthcare professionals to ensure comprehensive care and recovery.• <strong>Discharge & Follow-Up:</strong> Discharge patients when goals are met, providing recommendations for continued care or referrals as needed.• <strong>Other Duties:</strong> Perform other related tasks as assigned, ensuring flexibility in patient care as necessary.</p>\n<h2>Responsibilities</h2>\n<p><strong>What We’re Looking For</strong>• <strong>Qualifications:</strong> Licensed Physical Therapist with a valid state license.• <strong>Experience:</strong> Previous experience in a clinical or home health setting, especially with geriatric care, is preferred.• <strong>Skills:</strong> Strong assessment, communication, and organizational skills.• <strong>Patient-Centered Care:</strong> Empathy and the ability to work with patients facing various physical challenges.• <strong>Independence:</strong> Ability to work independently with a flexible schedule.• <strong>Tech Proficiency:</strong> Experience with electronic medical records (EMR) and Microsoft Office Suite.• <strong>Other:</strong> Must pass a criminal background check.</p>\n<h2>Pay Range</h2>USD $45.50 - USD $48.00 /Hr.",
