Home Support</strong></p><ul><li>Teach daily living skills in consumers’ homes and within the community.</li><li>Assist consumers with:    <ul>      <li>Health care needs</li>      <li>Finances</li>      <li>Cooking and meal planning</li>      <li>Shopping</li>      <li>Home care and household tasks</li>      <li>Personal care</li>      <li>Community education</li>    </ul></li><li>Model and educate consumers on healthy relationship skills and personal boundaries.</li><li>Advocate for consumers with medical, dental, legal, and other service providers as needed.</li><li>Monitor and support self-medication administration with proficiency.</li><li>Respond appropriately to consumer emergencies and provide emotional support when needed.</li></ul><p><strong>2. Community Support</strong></p><ul><li>Advocate for consumers with family, peers, and community members as appropriate.</li><li>Introduce consumers to community resources and assist them in accessing and using these services.</li><li>Encourage and support participation in community activities and events.</li><li>Teach safety skills relevant to both community and home settings.</li><li>Teach, instruct, and model appropriate social and community behavior and expectations.</li><li>Participate as an active member of the Supported Independent Living Program, Developmental Services, and UCS teams.</li></ul><p><strong>3. Record Keeping / Administrative</strong></p><ul><li>Submit daily timesheets using accurate service codes in the electronic medical record system.</li><li>Request and submit required documentation from other service providers and professionals.</li><li>Complete progress notes, monthly summaries, and all other documentation in a timely manner.</li><li>Maintain awareness of all consumer goals and supports outlined in the ISA and document progress accordingly.</li><li>Attend staff meetings, supervision sessions, and scheduled agency/division meetings as required.</li><li>Complete all mandatory trainings.</li></ul><p><strong>DSP Qualifications</strong></p><p><strong>Education &amp; Experience</strong></p><p>Required:</p><ul><li>High School Diploma or equivalent</li></ul><p>Preferred:</p><ul><li>Direct Service experience preferred</li><li>Experience with individuals with intellectual disabilities preferred</li></ul><p><strong>Skills</strong></p><ul><li>Proficiency in using Microsoft Outlook for internal e-mail communication</li></ul><p><strong>Licenses/Certifications &amp; Other</strong></p><ul><li>Valid Driver’s License</li><li>Reliable transportation to transport clients on occasion</li></ul>",
