The BHCM partners closely with primary care providers, psychiatric consultants, and interdisciplinary teams to support patients with common mental health and substance use disorders.</p>\n<p> </p>\n<p>This role emphasizes population-based care, systematic follow-up, registry-driven tracking, and brief interventions aligned with Collaborative Care Model standards. Responsibilities include:</p>\n<ul>\n <li>Support the mental and physical health care needs of patients on an assigned caseload.</li>\n <li>Coordinate care closely with medical providers and other mental health providers as appropriate.</li>\n <li>Screen and assess patients for common mental health and substance abuse disorders.</li>\n <li>Facilitate patient engagement, follow-up care, and provide education on treatment options.</li>\n <li>Track treatment response and monitor symptoms, side effects, and treatment adherence in person or by phone.</li>\n <li>Support psychotropic medication management as prescribed by medical providers.</li>\n <li>Provide brief evidence-based behavioral interventions, including behavioral activation, Cognitive Behavioral Therapy (CBT), Dialectical Behavioral Therapy (DBT)-informed skills, motivational interviewing, and solution-focused interventions.</li>\n <li>Provide or coordinate referrals to evidence-based psychosocial treatments and outside services, including specialty mental health care, substance abuse treatment, social services, and vocational rehabilitation.</li>\n <li>Participate in regular caseload consultations with psychiatric consultants and communicate treatment recommendations to medical providers.</li>\n <li>Track patient follow-up and outcomes using a registry/EHR system, documenting encounters, progress, and treatment recommendations.</li>\n <li>Facilitate treatment plan changes for patients not improving as expected in collaboration with providers and psychiatric consultants.</li>\n <li>Develop relapse prevention and self-management plans for patients preparing for discharge from the caseload.</li>\n <li>Other duties as assigned.</li>\n</ul>\n<h2>What We're Looking For:</h2>\n<ul>\n <li>Master’s-level degree in a behavioral health field required.</li>\n <li>Mental Health Licensure or Associate level licensure (e.g., LCSW(A), LCMHC(A), LMFT(A)) preferred.</li>\n <li>Experience in integrated care, primary care, or collaborative care settings preferred</li>\n <li>Experience with screening for common mental health and/or substance abuse disorders.</li>\n <li>Basic knowledge of psychopharmacology for common mental health disorders that is within appropriate scope of practice for type of provider filling role.</li>\n <li>Experience with evidence-based counseling techniques</li>\n</ul>\n<h2>About Blue Ridge Health:</h2>\n<p>At Blue Ridge Health our mission is to improve Health, inspire Hope, and advance Healing through access to Compassionate, Affordable, and Quality Care.
