The Coordinator, Community Services-Ryan White Program will provide clients and their families with ongoing community support resources with an emphasis on housing resources. The Coordinator, Community Services-Ryan White Program acts as an outstanding subject matter expert related to client treatment and housing needs and will foster positive relationships to all clients, their families, and Signature Health team members.</p><p><br></p><p><strong>HOW YOU’LL SUCCEED</strong></p><ul><li>Create a standout client care experience by effectively coordinating with the treatment team and linking clients to community resources.</li><li>Act as an effective subject matter expert when working with your team to assist other Infectious Disease team members in identifying resources to advocate for clients.&nbsp;</li><li>Develop and implement a housing care plan which indicates focus on housing information, referrals, and linkage to housing resources.</li><li>Act as a liaison with outside providers such as inpatient units and residential settings as well as internal care team members.</li><li>Assist HIV, Ryan White clients by identifying support needed in the following areas: educational, medical, vocational, housing, and other social determinants of health for clients to be successful in their communities.</li><li>Assist HIV, Ryan White clients by identifying support needed for housing resources, including access to help with utilities, rent, identifying affordable housing, etc.</li><li>Assist, educate, and support people with HIV regularly.</li><li>Assist, educate, and support people living with HIV on housing resources.</li><li>Conduct patient outreach and assist with coordination to recruit or re-engage existing clients who may have fallen out of care.</li><li>Assist in crisis intervention and prevention in the community.&nbsp;</li><li>Complete benefit analyses to aid clients in obtaining healthcare coverage.</li><li>Maintain client charting electronically in accordance with professional standards.</li><li>Maintain clear communication with referents or designated liaison person.</li><li>Link, encourage, monitor, and follow-up on client engagement and participation in all treatment plan activities.</li><li>Represent Signature Health Inc. at community events to provide linkage and education regarding SH services and community resources.&nbsp;&nbsp;</li><li>Participate in community outreach.</li><li>Participates in all quality assurance and utilization review activities.</li><li>&nbsp;Other duties as assigned.</li></ul>",
