Interprets, evaluates and reviews of physician inpatient orders and outpatient prescriptions in order to prepare, calculate, compound, admix and dispense appropriate medication and dosages consistent with optimum patient care. The interpretation, evaluation and review must include but is not limited to patient drug allergy review, drug-drug, drug-food interaction determination, assessment of therapeutic duplication, over/under dosing and inappropriate therapy and the prompt resolution and documentation of deficiencies found. (I.E., pharmacist is responsible to ensure the dispensation of the right medication, at the right dosage, for the right medical indication, at the right time, to the right patient.
