You help make life better by ensuring patients are informed and confident in their drug therapy—playing a vital role in their overall care and well-being.</p>\n<p> </p>\n<p>If you’re a pharmacist who understands the importance of your role in patient outcomes and values compassionate, high-quality care, you may be an ideal fit for this opportunity within the Legacy community.</p>\n<p> </p>\n<p>ED experience strongly preferred</p>\n<p> </p>\n<h2>Responsibilities</h2>\n<p><u>CLINICAL DUTIES</u><strong>:  </strong>Optimal patient care outcomes from drug therapy are ensured through clinical pharmacy activities. Such activities are age appropriate for neonates, pediatric patients, adolescents, adults, and geriatric patients, and in direct association with or in support of health care providers. The pharmacist will:</p>\n<ul>\n <li>Demonstrate clinical competence by completing the clinical competency modules assigned in the electronic learning system for the pharmacist role.</li>\n <li>Review and evaluate an individual patient’s medication profile and clinical status to determine appropriateness of current medication therapy.</li>\n <li>Ensure that a patient’s medication therapy is consistent with the Legacy or health insurance formulary and medication use guidelines and policies.</li>\n <li>Ensure that patients are assessed comprehensively in a standardized way using electronic health record (EHR) tools.</li>\n <li>Perform regular re<strong><em>-</em></strong>evaluation of medication therapy to assess whether therapeutic goals are achieved and to proactively intervene on medication related issues.</li>\n <li>Perform medication therapy management and monitoring based on Legacy specific Collaborative Practice Protocols (CPP).</li>\n <li>Document interventions, recommendations, and protocol driven changes in therapy in the EHR.</li>\n <li>Monitor, document, report, and manage adverse drug events.</li>\n <li>Provide accurate and timely medication information to health care staff and patients.</li>\n <li>Serve as a trainer for new pharmacy staff.</li>\n <li>Serve as a preceptor for pharmacy students and residents<strong><em>.</em></strong></li>\n <li>Perform other duties as assigned.</li>\n</ul>\n<p><strong><u>DISTRIBUTIVE DUTIES</u>:  </strong>Safe and accurate distributive services are ensured through standardized, efficient medication use processes. The pharmacist will:</p>\n<ul>\n <li>Ensure all medication orders are reviewed for appropriate dispensation during the assigned shift.</li>\n <li>Ensure appropriate use of applicable pharmacy technical systems, equipment (including environmental monitoring), and tools.</li>\n <li>Oversee preparation of medication doses and compounding of sterile and non-sterile products.</li>\n <li>Perform all assigned work procedures without interference to quality of work performed.</li>\n <li>Provide supervision to technicians, students and interns as guided by state law.</li>\n <li>Serve as a trainer for new pharmacy staff.</li>\n <li>Serve as a preceptor for pharmacy students and residents<strong><em>.</em></strong></li>\n <li>Use time productively to assist team members as workload allows.</li>\n <li>Fill outpatient prescriptions as required.</li>\n <li>Work with insurance companies/third party payers to resolve reimbursement and eligibility issues for patients where applicable.</li>\n <li>Perform other duties as assigned.</li>\n</ul>\n<p><strong><u>COMMUNICATION</u>:  </strong>The pharmacist will:</p>\n<ul>\n <li>Effectively communicate information to patients and health care providers in oral, written, and electronic formats.</li>\n <li>Foster teamwork through timely, open, and respectful communication.</li>\n <li>Demonstrate good listening skills.</li>\n <li>Provide education to patients.</li>\n <li>Provide excellent customer service.</li>\n</ul>\n<p><strong><u>PROFESSIONAL DEVELOPMENT</u>: </strong>Pharmacy knowledge and skills are updated, shared, and applied continuously to improve quality of pharmaceutical care. The pharmacist will:</p>\n<ul>\n <li>Maintain clinical and distributive knowledge and skills appropriate to the patient populations and areas served.</li>\n <li>Maintain knowledge of internal practice changes (e.g., drug shortages, new protocols) that may impact clinical and/or distributive services.</li>\n <li>Participate in preceptor education as applicable to precepting role.</li>\n <li>Maintain licensure in good standing with the state licensing body.</li>\n</ul>\n<p><strong><u>SAFETY AND SECURITY</u></strong>:  Patient, personnel, department, and facility safety and security are ensured through knowledge and application of established policies and procedures, and safety, security, and emergency plans. The pharmacist will:</p>\n<ul>\n <li>Follow all controlled substances policies and procedures.</li>\n <li>Complete annual mandatory self-learning modules that include safety, security and emergency programs required by the department or health system.</li>\n <li>Ensure a clean, safe, and orderly environment during the work shift.</li>\n <li>Comply with all laws, regulations, scope of practice guidelines, and Legacy standards in providing patient care.</li>\n</ul>\n<h2>Qualifications</h2>\n<p><strong>Education: </strong></p>\n<ul>\n <li>As required by licensure. </li>\n</ul>\n<h2>Licensure</h2>\n<ul>\n <li>Current Registered Pharmacist License (RPh) for applicable state required. </li>\n <li>BLS for Provider certification from the American Heart Association required within 3 months.</li>\n</ul>\n<h2>Pay Range</h2>USD $82.40 - USD $95.58 /Hr.\n<h2>Our Commitment to Health and Equal Opportunity</h2>\n<p>Our Legacy is good for health for Our People, Our Patients, Our Communities, Our World.
