We're looking for compassionate and experienced nurses to provide care for our clients in their homes. As a Home Care Nurse, you will be responsible for overseeing the quality, expert care of your assigned homecare patients. Starting rate for LPN: $26 per hour, RN: $30 per hour depending on experience and availability.</p>\n<h2>Responsibilities</h2>\n<p><strong><u>What You’ll Do:</u></strong></p>\n<p>∙Administer medication in accordance with standards of practice.</p>\n<p>∙ Encourage social\\rehabilitative skills and encourage self-care and independence. </p>\n<p>∙ Perform client treatments in accordance with Plan of Care </p>\n<p>∙ Assist the consumer in learning appropriate self-care techniques. </p>\n<p>∙ Prepare clinical and progress notes to document care provided. </p>\n<p>∙ Monitor, document accordingly and report any change in client condition to Clinical Director/Clinical Supervisor </p>\n<p>∙ Promote activities to achieve a safe and healthy environment. </p>\n<p>∙ Observe aseptic technique when performing client procedures/care. </p>\n<p>∙ Collaborate with other members of the clinical team to develop and improve the Plan of Care </p>\n<p>∙ Assist the client in achieving established goals in the Plan of Care developed by the RN. </p>\n<p>∙ Maintain productivity according to the standards of the agency. </p>\n<p>∙ Maintain license, CEUs and mandatory employment requirements. </p>\n<p>∙ Participate in client care meetings, in-services, orientation and staff meetings. </p>\n<p>∙ Provide service in accordance with agency’s philosophy, standards, policies and scope of practice ∙ Participates in Performance Improvement and adheres to HIPAA guidelines. </p>\n<p>∙ Other duties as assigned.</p>\n<h2>Qualifications</h2>\n<p><strong><em>Join Us and Be Part of Our Team!</em></strong></p>\n<p><strong><u>What You’ll Bring to the Team:</u></strong></p>\n<ul>\n <li>Valid State LPN/RN License</li>\n <li>Valid driver’s license and use of automobile is required due to the travel involved. </li>\n</ul>\n<ul>\n <li>Annual MUI (Incident) training as dictated by the State of Ohio Medicaid, Ohio Home Care Program and Department of Developmental Disabilities </li>\n <li>Must be able to read and understand various documents used by the agency including Plan of Care, flow charts, client charts, agency policies and procedures. </li>\n <li>Must be able to use a computer, laptop or tablet. </li>\n <li>Must be able to think independently, recognize potential problems, gather information, and know when to contact a supervisor. </li>\n <li>Employees must promote excellent customer service and always provide exceptional client care, while staying within the rules of the agency and rules of each program served. </li>\n</ul>\n<p> </p>\n<p> </p>\n<p><strong><u>Your Benefits Include, But Are Not Limited To:</u></strong></p>\n<p><strong>Benefits</strong></p>\n<p> </p>\n<ul>\n <li>Highly competitive salary.</li>\n <li>Health insurance</li>\n <li>Generous PTO</li>\n <li>Weekly Pay</li>\n <li>Flexible Schedule</li>\n</ul>\n<p> </p>\n<p><strong>Homecare With Heart</strong> is a locally based provider of home-based private duty, skilled nursing, personal care, pediatric nursing, physical therapy, as well as education and advocacy.
