Description

Description Redwood Coast PACE with Humboldt Senior Resource Center is looking for a full-time Center CarePartner to provide care for our participants. Redwood Coast PACE — a Program of All-inclusive Care for the Elderly — is a comprehensive health plan for older adults who wish to remain living in their home and community. We provide medical, social, nutritional, rehabilitation, and support services for eligible adults. At Redwood Coast PACE, the goal is to help people stay as healthy and independent as possible. Because their health is monitored on a regular basis, many PACE participants report fewer health crises and better overall health. The schedule for this position is Monday through Friday, 8:15 am to 3:15 pm. (32.5 hours a week) JOB SUMMARY: Under the supervision of the Therapeutic Services Coordinator, provides personal care services to participants in the Day Center and during transit to medical appointments according to departmental, para-professional, and regulatory standards of care. ESSENTIAL DUTIES AND RESONSIBILITIES: Assists participants who attend the Day Center with bathing, grooming, toileting, and feeding. Accompanies participants to medical appointments in the Clinic as needed. Routinely monitors vital signs such as weight, blood pressure, and pulse. Maintains personal care supplies for the Day Center. Coordinates and assist with meals, nutritional supplements and the monitoring of food and fluid intake in the Day Center according to each participant’s Plan of Care. Maintains a clean and sanitary environment in the Day Center at all times. Assists participants upon arrival in the Day Center and helps participants to prepare for departure when leaving. Facilitates both one-on-one and group activities (arts and crafts, music, discussion, and other recreational activities) with varied participant population. Assists participants with transfers, ambulation, and prescribed group exercises under the direction and supervision of the Physical and Occupational Therapists. Coordinates and performs tasks as directed in the task lists/calendar or under the direction of the Therapeutic Services Coordinator. Attends weekly meetings, monthly staff in-service trainings, and other meetings as requested. Immediately reports any observed changes in a participant’s behavior, health, or mobility to the Therapeutic Services Coordinator and other members of the Interdisciplinary Team as needed. Participates on the Interdisciplinary Team as required and communicates effectively with participants, family members, caregivers, employees, and visitors. Completes documentation in the medical record accurately and in a timely manner. Complies with safety policies and procedures, identifying and immediately reporting any potential or actual unsafe acts or conditions per the policies and procedures. Takes necessary measures to ensure a safety environment for oneself, co-workers, contractors, participants, visitors and others. Monitors personal safety and attends to good body mechanics. Ensures infection and exposure control and is prepared in case of emergency. Assists in the monitoring of Participant safety including knowledge of fall precautions, management of wandering participants and ability to use the delayed egress system. Consistently meets or exceeds targets for productivity and customer service. Continually seeks better ways for delivering services and communicating with participants. Effectively collaborates with co-workers and contractors to meet goals and further success of the programs. Complies with all policy and procedures. Demonstrates proficiency in delivering age-specific care. Participates in the process of teaching, training, and mentoring student interns and volunteers working within the program area. Acts in a manner consistent with agency values. Other duties as assigned. Qualifications EDUCATION &/or EXPERIENCE : Must have a high school diploma or GED equivalent. Must have at least one (1) year of experience working with a frail or elderly population, preferably providing personal care in a health facility or in-home setting. LICENSES OR CERTIFICATES : Current CPR and First Aid certification required within six (6) months of hire. Certified Nursing Assistant, Certified Home Health Aide, or Injections Certificate preferred (additional compensation may be available for certified individuals). PRE-SCREENING REQUIREMENTS: Requires clearance of a DOJ and FBI criminal history background check. Requires pre-employment physical, and PPD skin test (TB screening). COVID-19 and Influenza (flu) vaccines preferred. JOB SKILLS: Demonstrates respect for and promotes participant rights, including dignity, self-determination, access to care, confidentiality, and independence. Understands the importance of community involvement and participates as appropriate in activities that link our programs to the community. Ability to provide care for frail elderly. Excellent interpersonal interactions, teamwork, and problem-solving skills. Ability to complete duties within an agreed upon time frame and to adjust personal schedule if required. Ability to adapt appropriately to change of priorities and workload. Ability to maintain a consistent level of productivity. Sound verbal communication skills to convey information effectively and listening skills to receive directions and understand participant concerns. Ability to maintain a positive attitude and balance in relationships with others. Exhibits patience and compassion in work with participants. Ability to understand detailed requirements in working activities. Basic knowledge of computers and web-based applications such as timekeeping, training, and email programs. PHYSICAL REQUIREMENTS: The position requires frequent physical activity, including standing, walking, bending, stooping, kneeling, and reaching while assisting participants with activities of daily living. Employees must be able to safely lift, push, and pull up to 50 pounds on occasion, including when transferring or repositioning participants. The role involves fine motor skills for tasks such as meal preparation, vital sign monitoring, and documentation, as well as the ability to see, hear, and communicate clearly to ensure participant safety and quality care. Driving to participants’ homes and accompanying them to appointments may be required. This position also requires the ability to work in a variety of home environments and maintain infection control and safe body mechanics at all times.
