Responsibilities include : Complete skilled nursing visits during evenings and weekends Perform: Start of Care (SOC) assessments Resumption of Care visits PRN visits Urgent nursing visits beginning at 4:00 PM Respond to after-hours patient calls and triage concerns appropriately Coordinate care with: Physicians Case managers Interdisciplinary care teams Provide skilled clinical care including: Comprehensive patient assessments Medication reconciliation Wound care Disease management education Catheter care Support urology-related patient needs including : Foley catheter management Nephrostomy care Urinary retention monitoring Post-surgical urology follow-up Infection prevention education Complete documentation within required EMR timeframes Participate in rotating evening/weekend on-call coverage Schedule Structure Monday – Friday On-call availability: 4:00 PM – 8:00 AM Coverage may include: Start of Care admissions Late-add visits Urgent skilled nursing visits Telephone triage support Weekend Coverage (Rotating) Potential schedules include: Option 1 Saturday 8:00 AM → Monday 8:00 AM Option 2 Saturday: 8:00 AM–8:00 PM Sunday: 8:00 AM–8:00 PM Overnight triage unless an emergent visit is required Productivity Expectations Minimum weekly productivity: 18 points Point opportunities include: Start of Care admissions Skilled nursing revisits PRN urgent visits Resumption of Care visits Weekend precepting support Qualifications Active Florida RN license or Compact RN license Preferred experience: Home Health Urology Med-Surg Hospice Acute Care Strong assessment and triage skills OASIS documentation experience preferred Ability to work independently Reliable transportation, valid driver's license, and auto insurance Ideal Candidate You may be a great fit if you: Prefer flexible evening work Enjoy independent clinical decision-making Feel comfortable managing urgent patient needs Have strong Foley/urology management experience Want supplemental income with predictable compensation Apply Today – Start Your Home Health RN Career Join a team that values your time, supports your growth, and empowers you to provide exceptional patient care.
