As an LPN you will provide direct patient care under the supervision of a Registered Nurse, delivering treatments, assisting in assessments, and supporting interdisciplinary care planning in accordance with the patient's needs. Accurately performs patient assessments and identified patient needs. Identifies and initiates appropriate nursing interventions. Provides patient and family teaching throughout the rehabilitation stay and prior to discharge. Provides care appropriate to the condition and age of the patient.
