All Nurse Practitioners, Primary Care work collaboratively with Signature Health staff, including the behavioral health providers, counselors, case managers, and outside partners to achieve holistic, integrated care.</p><p><br></p><p><strong>HOW YOU’LL SUCCEED</strong></p><ul><li>Obtaining and legibly recording, a detailed medical history of each patient, including, but not limited to, the patient’s chief complaint, history of present illness, and past medical, social, and family history, all as medically appropriate.</li><li>Demonstrated clinical competence including physical assessment, health teaching and counseling, and make sound clinical judgments based on assessment of the physical, psychological, social and environmental needs of the client.</li><li>Prescribe treatment for illnesses within the Nurse Practitioner’s scope of practice, based on standing orders and nurse practitioner formulary.</li><li>Determine which problems are within scope of practice, and which require referral to a physician according to mutually accepted guidelines.</li><li>Analyze health behavior related to personality, life style, socio-economic status and culture.</li><li>Analyzing reports and/or findings of tests and personal examinations in order to assess the medical condition of the patient, including diagnosis and prognosis.</li><li>Referring the patient to other services within the Signature Health, other physicians, including specialists and other health care providers, when and if needed.</li><li>Prescribing medication and treatment for the patient that is consistent with standard clinical practice, and the APRN’s education, training, and scope of practice.</li><li>Performing such clinical procedures and therapeutic techniques as are consistent with APRN’s prior practice patterns and training.</li><li>Preparing and attending to all reports, claims, and correspondence necessary or appropriate to the performance of the professional services.</li><li>Attending professional conventions and post-graduate seminars as well as participating in professional societies on a reasonable basis.</li><li>Promoting the professional practice of Signature Health.</li><li>Providing coverage when other physician/APRN employees/contractors are unavailable due to vacations, holidays, illness, and attendance at professional activities.</li><li>Timely reporting of major unusual incidents and other information required to appropriate governmental authorities and as required for Signature Health reporting.</li><li>Participate in Signature Health quality assurance and peer review activities.</li><li>Performing other duties assigned by Signature Health from time to time that are commensurate with professional services normally and customarily performed by an APRN.</li><li>Being available to give coverage for other providers and taking client calls as assigned by Signature Health.</li><li>Following all policies and procedures of Signature Health.&nbsp;</li><li>Decision Making Authority: In providing primary care services, Nurse Practitioner, Primary Care will exercise independent medical judgment consistent with approved methods and practices in his/her specialty and in the best interest of the patients.
