Our staff are the heart of our mission, meeting clients where they are and building meaningful relationships that change lives.</p>\n<p><span style=\"font-weight: bold\"><br></span><br></p>\n<p><span style=\"font-weight: bold\"><span style=\"color: rgb(244, 124, 72)\">REPORTS TO</span></span></p>\n<p><br></p>\n<p><span style=\"color: rgb(34, 34, 34)\">Waiver Services Supervisor</span></p>\n<p><br></p>\n<p><span style=\"font-weight: bold\"><span style=\"color: #f47c48; font-size: 16px\">POSITION OBJECTIVE </span></span></p>\n<p><br><span style=\"font-size: 16px\">The primary responsibility of the Behavior Consultant will be to help clients identify and address problem behaviors through the creation and implementation of a Functional Assessment and Behavior Support Plan.  The goal of the Behavior Consultant is to decrease maladaptive behaviors by working with the client and his/her team to teach appropriate replacement behaviors. Behavior Consultant will work with individuals to teach positive replacement behaviors as a means to support and sustain ways they can live actively and independently within their communities. The Behavior Consultant will report directly to the Program Manager who may make changes in job duties from time to time.</span></p>\n<p><br></p>\n<p><span style=\"font-weight: bold\"><span style=\"color: #f47c48; font-size: 16px\">ESSENTIAL DUTIES AND RESPONSIBILITIES</span></span></p>\n<ul>\n<li><span style=\"font-size: 16px\">Completion of behavioral consultation with clients, family members and/or guardians and any other members of the client’s treatment team (such as: physicians, therapists, etc.).</span></li>\n<li><span style=\"font-size: 16px\">Completion of Functional Assessments (FA), Behavior Support Plans (BSP), and/or Human Rights Committee(HRC) Forms. </span></li>\n<li><span style=\"font-size: 16px\">Attendance and participation in Team meetings.  </span></li>\n<li><span style=\"font-size: 16px\">Attending any meetings/trainings scheduled by Program Manager, HSPP or Sky Point.</span></li>\n<li><span style=\"font-size: 16px\">Obtain all needed signatures for required paperwork including intake/annual documents.</span></li>\n<li><span style=\"font-size: 16px\">Responsible for bringing paperwork to the meeting for the purpose of filing copies in the main file. </span></li>\n<li><span style=\"font-size: 16px\">Responsible for making sure clients’ files are current and up-to-date both in the main file and the home file.</span></li>\n<li><span style=\"font-size: 16px\">Completion of training on the BSPs and behavior documentation procedures with clients, family members and other providers as needed and filing completed trainings in the main file.</span></li>\n<li><span style=\"font-size: 16px\">Completion of daily progress notes within a 48-hour time frame. </span></li>\n<li><span style=\"font-size: 16px\">Completion of monthly report for each client no later than the 1st day of the month at 8am for the previous month.  </span></li>\n<li><span style=\"font-size: 16px\">Submit/upload monthly reports no later than the 1st day of each calendar month.</span></li>\n<li><span style=\"font-size: 16px\">Ensure all billing/payroll units and hours are accurate prior to submission for billing/payroll.</span></li>\n<li><span style=\"font-size: 16px\">Consistent home visitation and follow up with service providers and family.</span></li>\n<li><span style=\"font-size: 16px\">Exhibit effective correspondence and communication with all providers, family members, staff members, clients and all other treatment team members as applicable. Be sure to return phone calls, text messages, and emails within a 24-hour time frame.</span></li>\n<li><span style=\"font-size: 16px\">Maintain a commitment to professional development in compliance with continuing education requirements as prescribed by Medicaid Waiver Program which dictates maintaining at least 10 hours of CEUs yearly.</span></li>\n<li><span style=\"font-size: 16px\">Carry and maintain current professional liability insurance.</span></li>\n<li><span style=\"font-size: 16px\">Adhere to professional and ethical interaction policies and procedures.</span></li>\n<li><span style=\"font-size: 16px\">Responding to crisis calls as soon as possible.</span></li>\n<li><span style=\"font-size: 16px\">Experience with Medicaid Waiver is helpful though we will train motivated individuals who have experience working with persons with developmental disabilities</span></li>\n<li><span style=\"font-size: 16px\">Position requires travel to consumer’s homes, must be willing to use personal vehicle and provide proof of current automobile insurance and registration</span></li>\n<li><span style=\"font-size: 16px\">Position consists of growing a caseload as referrals are received</span></li>\n</ul>\n<p><br></p>\n<p><span style=\"font-weight: bold\"><span style=\"color: #f47c48; font-size: 16px\">QUALIFICATIONS AND EDUCATION</span></span></p>\n<ul>\n<li><span style=\"font-size: 16px\">Possess a <span style=\"font-weight: bold\">Master’s degree</span> in Psychology, Sociology, Social Work, Special Education, Rehabilitation Counseling, Mental Health Counseling, Marriage and Family Therapy, or Applied Behavior Analyst.</span></li>\n<li><span style=\"font-size: 16px\">Must possess a commitment to the belief that all people have the right to dignity, respect, opportunity and full community inclusion.</span></li>\n<li><span style=\"font-size: 16px\">Must have a philosophy that is consistent with the Mission, Vision, and Core Values of the organization</span></li>\n<li><span style=\"font-size: 16px\">A report of “no findings” when criminal history checks are completed.</span></li>\n<li><span style=\"font-size: 16px\">Prior experience with persons with developmental disabilities required.</span></li>\n<li><span style=\"font-size: 16px\">Must possess a valid driver’s license, automobile registration and insurance.</span></li>\n</ul>",
