The LADC/MAT Clinical Care Coordinator is responsible for the coordination and care of patients receiving Medication Assisted Treatment (MAT) for opioid addiction. The position provides health home services in coordination with prescribing buprenorphine providers within the NOTCH Service Area. The LADC/MAT Clinical Care Coordinator is responsible for the coordination and care of patients receiving Medication Assisted Treatment (MAT) for opioid addiction. The position provides health home services in coordination with prescribing buprenorphine providers within the NOTCH Service Area. This position will provide for the planning, coordination, and transition of care as patients move along the healthcare spectrum.\\n\\n \\n\\nESSENTIAL DUTIES AND RESPONSIBILITIES\\n\\nThe essential functions include, but are not limited to the following:\\n\\n\\n\\tComplete and maintain accurate records or reports regarding the patients’ histories and progress, services provided, or other required information.\\n\\tCounsel patients, individually or in group sessions, to assist in overcoming dependencies, adjusting to life, or making changes.\\n\\tDevelop client treatment plans based on research, clinical experience, and client histories.\\n\\tParticipate in case conferences or staff meetings.\\n\\tCoordinate counseling efforts with mental health professionals or other health professionals, such as doctors, nurses, or social workers.\\n\\tReview and evaluate client’s progress in relation to measurable goals described in treatment and care plans.\\n\\tPlan or implement follow-up or aftercare programs for clients to be discharged from treatment programs.\\n\\tAssess individuals’ degree of drug dependency by collecting and analyzing urine samples. \\n\\n\\n \\n\\nKNOWLEDGE, SKILLS, AND ABILITIES \\n\\n\\n\\tKnowledge of current behavioral medicine approaches.\\n\\tAbility to utilize appropriate, proven intervention methods.\\n\\tUnderstanding of mind, body and bio psychosocial implications of health.\\n\\tStrong written/verbal skills. \\n\\tAbility to complete all written paperwork in a timely manner (written assessments, plans, progress notes, etc.).\\n\\tGood organizational and problem-solving skills.\\n\\tExperience with addictions assessment and treatment.\\n\\tAbility to present in a professional manner.\\n\\tKnowledge of human behavior and performance; individual differences in ability, personality, and interests; learning and motivation; psychological research methods; and the assessment and treatment of behavioral and affective disorders.\\n\\tKnowledge of the principles, methods, and procedures for diagnosis, treatment, and rehabilitation of physical and mental dysfunctions, and for career counseling and guidance.\\n\\tKnowledge of group behavior and dynamics, societal trends and influences, human migrations, ethnicity, cultures and their history and origins.\\n\\tStrong diagnosis and assessment skills, psychotherapy, case management, and crisis intervention skills.\\n\\tKnowledge of mental health and substance abuse laws and regulations.\\n\\tAbility to obtain and maintain credentials and privileges as a NOTCH Licensed Alcohol and Drug Counselor.\\n\\tIntermediate Microsoft Office suite skills required.\\n\\n\\n \\n\\nEDUCATION\\n\\nMaster’s Degree in Social Work\\n\\n \\n\\nEXPERIENCE\\n\\nTwo years in addictions field preferred\\n\\nTwo years in mental health field preferred\\n\\n \\n\\nCERTIFICATE/LICENSE\\n\\nVermont licensure as a Licensed Alcohol and Drug Abuse Counselor (LADC)\\n\\nBasic Life Support (BLS) certification required or ability to obtain certification\\n\\n \\n\\nPHYSICAL DEMANDS AND WORK ENVIRONMENT\\n\\nThe physical demands described here are representative of those that must be met by an employee to successfully perform the essential functions of this position.
