Description

Overview At the direction of the Medical Director, the Medical Clinic Manager (Nurse Manager – Primary Care) provides supervision, direction, and leadership to medical department support staff and is responsible for the day to day clinical and operational management of the primary care clinic. This role ensures safe, efficient, culturally responsive, and compliant delivery of primary care services, including nursing care, patient triage, population health, care coordination, and quality improvement efforts. Responsibilities Supervises medical department staff in accordance with Santa Ynez Tribal Health Clinic (SYTHC) policies, practice guidelines, and applicable state and federal laws. Hires, trains, develops, empowers, coaches, counels, evaluates performance and compensation, disciplines, and recommends termination of supervised staff as appropriate. Builds and sustains a work environment that promotes teamwork, collaboration, accountability, recognition, and professional growth while role modeling the One.Team.Chumash values. Ensures adequate staffing levels and appropriate provider-to-staff ratios to support patient access, continuity of care, and safe clinical operations. Approves timesheets, leave, and time-off requests, and coordinates staffing schedules based on patient care demand. Oversees and optimizes primary care workflows, including rooming standards, standing orders, nursing protocols, scope-of-practice utilization, and team-based care models. Ensures implementation of and adherence to evidence-based clinical guidelines for preventive care, chronic disease management, immunizations, and screenings within scope of practice. Oversees patient triage processes, same-day access workflows, and clinical support for acute and chronic visits. Monitors clinic flow, appointment access, cycle time, and no-show rates to improve efficiency and patient experience. Collaborates with providers and front office staff to maintain efficient, patient-centered clinic operations. Oversees population health management activities, including preventive outreach, chronic disease registries, and improvement of care metrics. Supervises and participates in care management for diabetic, complex, and high-risk patients in collaboration with Providers and Community Services. Oversees care coordination workflows, including tracking, follow-up, and transitions of care following hospital or emergency department encounters. Collaborates with the referrals team to ensure provider-ordered referrals are completed and clinically closed, including confirmation of consult outcomes, documentation in the EHR, and communication back to the ordering provider. Participates in diabetic team meetings and supports oversight of diabetic grant activities and reporting requirements. Ensures compliance with OSHA, CLIA, AAAHC, infection prevention standards, and all patient and staff safety requirements. Works closely with the Medical Director to conduct audits, monitor compliance, develop corrective action plans, and prepare for AAAHC surveys. Participates actively in the Quality Improvement Committee, Safety Committee, and Diabetic Committee. Leads or supports quality improvement initiatives using data-driven methodologies to improve primary care outcomes and patient experience. Coordinates GPRA performance data collection and reporting using the Eagle Dream system and submits required reports to the Indian Health Service (IHS). Provides routine GPRA and quality performance updates to the Medical Director and leadership. Maintains functional expertise in the NextGen electronic health record and collaborates on EHR optimization to support primary care workflows, templates, registries, and health maintenance tools. Serves as liaison between the medical department and NextGen support services and educates staff on system updates and enhancements. Orders and maintains medical department supplies and equipment in accordance with the Medical Director’s approved budget. Maintains working knowledge of all medical department programs and funding sources, including Medi-Cal, VFC, and other applicable state and federal programs. Presents departmental updates and reports to the Executive Director and/or Clinical Director. Performs other duties as assigned. Qualifications Bachelor’s degree in Nursing or in a related field. Registered Nurses license valid in California. Current and valid CPR certification. Five years of experience in the medical field with progressive responsibility. Intermediate computer proficiency utilizing Microsoft applications, email, and internet. Bilingual in English and Spanish preferred. Ability to read, analyze, and interpret program forms, grants, and documents. Native American hiring preference applies. Organizational Awareness: Having and using knowledge of systems, situations, procedures, and culture inside the organization to identify potential problems and opportunities; perceiving the impact and the implications of decisions on other components of the organization. Leadership: Achieving results through people by successful objective setting, performance review, motivation, delegation, teambuilding, commitment gains, and empowerment. Technical Expertise and Usage: Acquiring and applying technical and functional knowledge in a technological area of specialty.
