What You’ll Be Doing
You will play a key role in coordinating care and improving patient outcomes through both direct engagement and data-driven decision making:
Track patient progress during the first 90 days of treatment and intervene as needed
Conduct proactive outreach to patients (calls, letters, follow-ups)
Coordinate care with internal teams and external partners (jails, SNFs, community providers)
Monitor key data points such as missed doses, UAs, and treatment adherence
Support Medication First Track patients and assist with panel management
Participate in interdisciplinary team meetings, huddles, and care planning
Coordinate referrals for housing, primary care, mental health, and other services
Maintain accurate documentation in the electronic medical record
Ensure compliance with regulatory standards (OARs, Joint Commission, SAMHSA)
Support development of dashboards and data tracking to improve outcomes

Qualifications
What You Bring
2+ years experience in care coordination, healthcare, or social services
Strong understanding of community resources and patient engagement strategies
Ability to analyze data and translate it into action
Strong communication and relationship-building skills
Certifications (Required or Within Timeline):
CADC I (or ability to obtain within 24 months; CADC-R within 30 days minimum
CPR Certification (within 90 days)
Crisis Prevention Training (within 90 days)
Valid OR/WA Driver’s License (if required for role)
