Join us in making a real difference in the health of our community.</em></p>\n<h2>Job Summary</h2>\n<p><strong>Position Title:</strong> Licensed Master Social Worker- Hybrid opportunities</p>\n<p><strong>Reporting to:</strong> Office Division</p>\n<p><strong>Job Summary:</strong> The Licensed Master Social Worker will be responsible to perform daily social work operations</p>\n<p> </p>\n<h2>Responsibilities</h2>\n<ul>\n <li>Assessment of patients referred by Primary Care Provider (PCP) using evidenced based assessment tools. </li>\n <li>Treatment of behavioral health conditions using evidenced based approaches (CBT, Motivational Interviewing, crisis intervention, Interpersonal Psychotherapy, and behavioral activation). </li>\n <li>Make treatment recommendations during collaborative care team meetings</li>\n <li>Monitor treatment through care planning and coordination of care with clinical team and health homes. </li>\n <li>Provide crisis intervention services and risk assessment of patients as needed. </li>\n <li>Engage clients with limited utilization of behavioral health service through outreach efforts.</li>\n <li>Document services in a timely fashion. </li>\n <li>Engage in weekly clinical supervision </li>\n <li>Engage in team meetings and case conferences as needed. </li>\n</ul>\n<h2>Qualifications</h2>\n<ul>\n <li>LMSW Licensure </li>\n <li>Master of Social Work Degree from accredited institution</li>\n <li>Ability to utilize behavioral health screening tools and perform biopsychosocial assessments</li>\n <li>Utilize DSM-V diagnostic criteria to diagnoses and treat behavioral health conditions</li>\n <li>Assess for risk and collaborate with psychiatric and clinical team around risk management</li>\n <li>Ability to utilize electronic health records for documentation of clinical services and review of relevant health data. </li>\n <li>Communicate clinical information in a succinct and comprehensive way to integrated care teams, inclusive of PCP, NP/MD, and Health Home Care Management staff.</li>\n <li>Bilingual Spanish strongly preferred</li>\n</ul>\n<p>Salary: $67,000-$80,000</p>\n<h2>Equal Opportunity Employer</h2>\n<p><strong>ESSEN HEALTH CARE IS PROUD TO BE AN EQUAL OPPORTUNITY EMPLOYER</strong></p>\n<p><em>Essen Health care is proud to be an equal opportunity employer, and we seek candidates who desire to work in and serve an ethnically diverse population.</em></p>",
