    "requisitionDescription": "<div><div><div><div><p data-pasted=\"true\"><strong>Housing Case Manager</strong></p><p><strong>Rapid Re-Housing &amp; SSVF Programs</strong></p><p>At Father Joe&rsquo;s Villages, housing is more than a destination &mdash; it&rsquo;s the foundation for stability, healing, and hope. The Housing Case Manager supports veterans, individuals, and families in obtaining and maintaining permanent housing while building long-term housing stability. You will work directly with clients to develop Housing Stability Plans, connect clients to resources, and provide ongoing support to help them achieve self-sufficiency. This role operates primarily out of the Veterans Village of San Diego (3330 Kemper Street, San Diego, CA 92110) and also at Father Joe&rsquo;s Villages (15th &amp; Commercial, San Diego, CA 92101).</p><p><strong>The Role</strong></p><p>As a Housing Case Manager, you will guide clients through the housing process, provide direct case management services, and connect clients to internal and external resources. This role is ideal for someone who is resourceful, relationship-driven, and passionate about helping clients achieve housing stability.</p><p><strong>Your Role in Supporting Housing Stability</strong></p><ul style=\"list-style-type: disc;margin-left: 0in;\"><li style=\"margin-left:0in;\">Conduct initial and ongoing assessments of client needs, progress, and barriers to housing stability.</li><li style=\"margin-left:0in;\">Develop and implement Housing Stability Plans in collaboration with clients to support long-term housing success.</li><li style=\"margin-left:0in;\">Maintain accurate case documentation in HMIS and other required systems in compliance with agency policies and grant requirements.</li><li style=\"margin-left:0in;\">Perform crisis intervention and provide direct support to clients navigating housing challenges.</li><li style=\"margin-left:0in;\">Identify and secure housing opportunities, including Fair Market Rate and other affordable options, for clients.</li><li style=\"margin-left:0in;\">Maintain and update a Housing Directory, recording all interactions with landlords and housing providers.</li><li style=\"margin-left:0in;\">Negotiate leases and coordinate housing placements in collaboration with internal teams.</li><li style=\"margin-left:0in;\">Conduct unit inspections to ensure compliance with program and housing standards.</li><li style=\"margin-left:0in;\">Educate clients on tenant rights, responsibilities, and effective communication with landlords.</li><li style=\"margin-left:0in;\">Collaborate with staff, attend SSVF and internal program meetings, and share relevant client updates.</li><li style=\"margin-left:0in;\">Provide ongoing follow-up to support clients in overcoming housing barriers and maintaining stability.</li><li style=\"margin-left:0in;\">Drive to client locations, housing sites, and partner offices as needed to support housing placements and client follow-up.</li><li style=\"margin-left:0in;\">Complete all required trainings and perform other duties as assigned by FJV and VVSD supervisors.</li></ul><p><strong>Minimum Requirements</strong><strong>&nbsp;</strong></p><div style=\"margin-left:0in;\"><ul style=\"list-style-type: disc;margin-left: 0in;\"><li style=\"margin-left:0in;\">High school diploma, GED, or equivalent combination of education and relevant experience.</li><li style=\"margin-left:0in;\">Two (2) years of experience in social services, human services, housing, or related field, with demonstrated ability to work with underserved populations.</li><li style=\"margin-left:0in;\">Knowledge of tenant rights, housing resources in San Diego, and landlord negotiation strategies preferred.</li><li style=\"margin-left:0in;\">Strong written communication skills and ability to maintain accurate client documentation.</li><li style=\"margin-left:0in;\">This role requires regular driving between worksites and client locations.
