    "ExternalDescriptionStr": "<p><strong>Job Summary:</strong> The Care Coordinator works to improve patient outcomes and satisfaction by coordinating and facilitating patient care across a multidisciplinary care continuum while ensuring compliance with associated requirements. Coordinates assessment, planning, and implementation of health care and related services to assigned patients. Additionally, we are honored to be named one of America’s Best-In-State Hospitals by <i>Newsweek</i>, among numerous other prestigious accolades.</p>\n<p><strong>Minimum Requirements:&nbsp;</strong></p>\n<ul>\n <li><span>Associate’s&nbsp;Degree&nbsp;or&nbsp;Bachelor’s&nbsp;Degree&nbsp;in&nbsp;Nursing;&nbsp;or&nbsp;Bachelor’s&nbsp;Degree&nbsp;in Social&nbsp;Work;&nbsp;or&nbsp;Bachelor’s&nbsp;Degree&nbsp;in&nbsp;Human&nbsp;Services/Social&nbsp;Services&nbsp;field with 3-5 years relevant hospital social service work experience.&nbsp;</span></li>\n <li><span>If nursing degree, must have current N.C. license.&nbsp;</span></li>\n <li><span>Three to 5 years of experience in acute&nbsp;health&nbsp;care&nbsp;setting&nbsp;preferred.</span></li>\n <li><span>&nbsp;Possesses the ability to relate effectively to patients, their families, physicians,&nbsp;staff&nbsp;and&nbsp;all&nbsp;others,&nbsp;using&nbsp;proper&nbsp;channels&nbsp;of&nbsp;communication.&nbsp;</span></li>\n <li><span>Ability and willingness to perform all job functions in compliance with CarolinaEast&nbsp;Health&nbsp;System&nbsp;and&nbsp;regulatory&nbsp;safety&nbsp;standards&nbsp;and&nbsp;insures the&nbsp;safety&nbsp;of&nbsp;patients,&nbsp;self,&nbsp;visitors&nbsp;and&nbsp;other&nbsp;employees.&nbsp;</span></li>\n <li><span>Must be able to demonstrate the knowledge and skills necessary to provide services according to the age of the patient population served.&nbsp;</span></li>\n <li><span>The individual must demonstrate knowledge of the principles of growth and&nbsp;development&nbsp;over&nbsp;the&nbsp;life&nbsp;span&nbsp;and&nbsp;possess&nbsp;the&nbsp;ability&nbsp;to&nbsp;assess&nbsp;data reflective of the patient's status and interpret the appropriate information needed&nbsp;to&nbsp;identify&nbsp;each&nbsp;patient's&nbsp;requirements&nbsp;relative&nbsp;to&nbsp;his/her&nbsp;age.</span></li>\n <li>Deliver outstanding customer service that upholds CarolinaEast's Standards of Excellence.</li>\n</ul>",
