Description

Overview Practices and promotes the agency’s mission statement in every interaction. Assists the Manager in maintaining and supervising Direct Support Professional Staff to ensure they are supporting people to become active participants in every aspect of their lives. Provides direct support as dictated by house staffing pattern and/or house needs. Values and Embraces Diversity in background and experiences for every person in every community. Strive to create an open, inclusive environment in which every employee has the opportunity to flourish. Demonstrates a commitment to understanding cultural differences and supports a culture of diversity, equity and inclusion. Responsibilities Practices and promotes the agency’s mission statement in every interaction Through family and community engagement, Citizens supports people to live the life they choose. Assists Manager with the supervision of Direct Support Professional Staff by assigning responsibilities and specific activities to provide and encourage each person to strive for autonomy and support them to become active participants in every aspect of their lives. Reports to the Justice Center as a mandated reporter if there is reasonable cause to suspect abuse or neglect. Additionally, must follow all agency incident policy and procedures. Reports and documents any events/activities that may hinder the health, safety and well-being of the people that reside in that house (i.e., allegations of abuse, injuries, broken equipment, etc.). Adheres to Federal and New York State regulations, agency policy and procedures, the employee handbook and the agency and program training requirements. Successfully completes and maintains AMAP certification and assists the people supported with medication administration according to AMAP policy and procedures. Assists with the maintenance of appropriate records of staff by observing and providing feedback on job performance, documenting issues and informing them of training needs to ensure the ongoing and effective supervision of each employee. Ensures a safe and clean home by providing direction to staff to support people with the completion of household chores and by participating in said household chores as needed. Assists Manager with the completion of daily rounds to ensure the presence, working order and cleanliness of all equipment (i.e. bed equipment, adaptive equipment, furniture and other equipment) needed for the people supported. Assists Manager with conducting physical plant walkthroughs and reports, documents and ensures timely corrective action for any concerns that may hinder the health, safety and well-being of the people that reside in that house (i.e., broken equipment, broken furniture, hazardous conditions, etc.). Provides direct support as scheduled on the staffing pattern and as needed to cover a vacancy or call off via flexing schedule to best meet the minimum staffing requirements of the people supported. Arrives to work by the start of scheduled shift and adhere to agency policy and procedure in regard to call offs. Assists Manager with the management of overtime to ensure quality support is provided to people each day in the most cost effective manner. Accompanies and/or transports people on medical appointments, shopping, recreational outings, programs and any other outing and/or assists with the arrangement of transportation for people by ensuring availability of staffing and agency vehicles to ensure the successful completion of required medical appointments and outings. Assists Human Resources with the recruitment process of new employees by assisting with interviews to ensure that the house is adequately staffed with qualified employees. Assists with the maintenance of weekly work schedules for all staff by assigning shifts based on availability and the particular requirements of each person to ensure that appropriate support is provided at all times. Assists with the maintenance of peoples' bank accounts and expenditures by depositing and withdrawing all monies with the person and reviewing monthly bank statements to ensure accuracy and adherence to individual budgets. Serves as a member of the inter-disciplinary team by attending and participating in all meetings to discuss peoples’ progress and to offer and gather information and ideas. Supervises staff to ensure that goals are implemented and documented whether it be via Medisked or paper documentation to include goals, notes, BM and menses charts, transportation logs, etc. Assists Manager with ensuring coordination of supports and services and ongoing communication with day service providers. Performs other duties as assigned by supervisor. This position requires regular and substantial unsupervised or unrestricted physical contact with people receiving services and, therefore, is subject to successful completion of fingerprint/criminal background check. Qualifications High School Diploma or GED preferred; a valid New York State Driver’s License that is also qualified in accordance with agency’s insurance standards; one year’s experience working with people with ID/DD; good oral and written communication skills; successful completion and maintenance of AMAP status; good organizational and administrative skills and an excellent team player with a positive attitude.
