    "description": "<p><span><span style=\"font-weight: bold\">Full Time Position</span><br>Monday - Friday and occasional Saturday admissions<br></span></p>\n<p><br></p>\n<p><span>Full Time Benefits:</span></p>\n<p><br></p>\n<ul>\n<li><span>Mentoring for New Professionals </span></li>\n<li><span>Paid Holidays </span></li>\n<li><span>Paid Sick Days</span></li>\n<li><span>Paid Vacation Days</span></li>\n<li><span>Retirement 401k and/or Roth IRA with company matching</span></li>\n<li><span>AD&amp;D Insurance</span></li>\n<li><span>Medical</span></li>\n<li><span>Dental &amp; Vision Plans</span></li>\n<li><span>Life Insurance</span></li>\n<li><span>Disability Insurance</span></li>\n<li><span>Malpractice Insurance</span></li>\n<li><span>Licensure Assistance</span></li>\n<li><span>Mileage Reimbursement</span></li>\n</ul>\n<p><br></p>\n<p><span>The Physical Therapist provides prescribed treatments of physical therapy, which involve a complex and sophisticated level of modalities designed to restore the patient to his/her maximum rehabilitative potential.<br><br>Identifies the problem and performs physical therapeutic treatments as prescribed by the patient's physician.<br><br>Assesses, reports, and records patient's status and progress on appropriate Agency laptop and to the health care team. <br><br>Ensures initial and on-going assessments to determine level of functioning and evaluation of patient/family response to therapy, including OASIS assessments at appropriate time points.<br><br>Plans, evaluates, implements and teaches patients and family members to provide for a continuous physical therapy program during care and discharged.<br><br>Participates in clinical conferences and in-service programs to share information and stimulate the health care team in meeting rehabilitation goals.<br><br>Delivers quality treatment services to patients in accordance with accepted standards of practice and effective for the condition, which the patient suffers.<br><br>Develops and revises a plan of treatment and care as indicated in collaboration and consultationwith the physician, ITC members, and other healthcare team to achieve goals that is not a duplication of active physical therapy programs.<br><br>Evaluates the effectiveness and outcomes of care and identifies barriers to desired outcomes. <br><br>Set goals related to the needs of the patient.<br><br>Maintains specialized hospice training, if necessary.<br><br>Schedules visits with patients according to assigned caseload in geographic location.<br><br>Prepares and maintains appropriate clinical and administrative records in a prompt and comprehensive manner, documenting patient limitations/interventions and progress. <br><br>Timely records all assessment and evaluation data, treatments and patient's response to therapy intervention. C<br><br>Completes paperwork within 24 hours of visit, and communicates daily.<br><br>Collaborates with Clinical Director to meet minimal productivity of 85%.<br><br>Develops and instructs Agency staff and family members in maintenance therapy programs and therapeutic treatments as approved by the physician.<br><br>Supervises and directs Licensed Physical Therapy Assistants and home health aides as appropriate in carrying out the plan of treatment as established by the therapist. <br><br>Assists with the orientation of new staff as needed.<br><br>Evaluates equipment needs to increase patient's functional and independence levels and assumes responsibility for equipment signed out for patient use.<br><br>Will be expected to travel to other offices to meet the demand of patient needs as instructed by Clinical Director.<br><br>Ensures referrals are evaluated timely with NPU goal of 6% or less.<br><br>Communicates daily with the Clinical Director or Team Leader (RN).<br><br>Attends appropriate number of inservices as required.<br><br>Adheres to the Agency's personnel policies and Compliance Plan.<br><br>Willingly performs other duties as assigned by supervisor.<br><br>Displays a high degree of courtesy, tact, and knowledge of services provided by the agency in all contact with staff, patients, and visitors.<br><br><span style=\"font-weight: bold\">JOB REQUIREMENTS </span></span></p>\n<p><span>Graduate of an approved Physical Therapy curriculum.<br><br>One-year of experience as a Physical Therapist required.<br><br>Physical Therapist currently licensed to practice physical therapy in North Carolina.<br><br>Maintain current CPR certification (BCLS)   </span></p>\n<p> </p>",
