The incumbent will assess, plan, evaluate, and deliver nursing care in collaboration with RN and/or Physician. This work is carried out in support of the mission and goals of Samaritan Daytop Village. </p>\n<h2>Responsibilities</h2>\n<p><strong>What You Will Do</strong></p>\n<ul>\n <li>Assists with patient care including checking vital signs, blood pressure, pulse rate, breathing, temperature, weight, discussing chief health concerns, administers injections and performs test.</li>\n <li>Takes patient’s medical history, all pretreatment formalities and enters data in computer system.</li>\n <li>Follows orders of physician and registered nurse in providing care.</li>\n <li>Records in patients’ record a detailed report of care given.</li>\n <li>Handles proper charting, documentation and maintenance of patients’ medical records.</li>\n <li>Checks, cleans and dresses wounds.</li>\n <li>Reinforces instructions given to patient by RN or Physician.</li>\n <li>Notifies RN or Physician of significant finding or changes in patient’s physical or mental condition.</li>\n <li>Places phone calls to pharmacies to order prescriptions.</li>\n <li>Maintains confidentiality of patient records.</li>\n <li>Pulls and prepares patient charts.</li>\n <li>Maintains neatness of work area.</li>\n <li>Stocks patient rooms with supplies.</li>\n <li>Provides emotional support as needed.</li>\n <li>Initiates emergency assistance as per policies and procedures.</li>\n <li>Attends staff development trainings and nursing conferences.</li>\n <li>Keeps skills current and increases knowledge and productivity.</li>\n <li>Performs other duties as requested.</li>\n</ul>\n<h2>Qualifications</h2>\n<p><strong>Who You Will Be</strong></p>\n<ul>\n <li>NYS LPN licensure.</li>\n <li>BLS certification.</li>\n <li>High School or GED. </li>\n <li>Graduate of a formal Technical or Vocational LPN Program.</li>\n <li>At least One (1) year experience as an LPN working with clients with Substance Use Disorder (SUD) and/or Serious Mental Illness (SMI).</li>\n <li>Available to work a flexible schedule as needed in response to participant and staff needs. </li>\n <li>Computer literacy including proficiency in Microsoft Office Suite and EHR.</li>\n <li>Ability to put patients at ease and explain physician’s instructions.</li>\n <li>Ability to multi-task with excellent organization skills.</li>\n <li>Experience working directly with people from diverse racial, ethnic and socioeconomic backgrounds.</li>\n <li>Knowledge of medical terminology.</li>\n <li>Team player and ability to work independently.</li>\n</ul>\n<p> </p>\n<p> </p>\n<p>#li-onsite</p>",
