POSITION SUMMARY:
This position is responsible for working as part of a multidisciplinary treatment team to coordinate clinical and case management services for adults who participate in Perception Programs (PPI) medication-assisted treatment & statewide opioid response (MAT-SOR) programs. This position will be the primary point of contact for the Mobile Assisted Treatment (MAT) van. Primary duties include driving, canvassing, and case management.
REPORTS TO:
Senior Director of Community Services
ESSENTIAL DUTIES AND RESPONSIBILITIES:
Facilitate communication with PPI providers and other clinical staff on new referrals, inductions, and discharges. Assist in coordination of physical examinations with primary care providers. Provide client education regarding available medication-assisted therapies. Ensure documentation and reporting is completed and submitted to meet agency, grant, contractual, and regulatory requirements and within designated timeframes; maintain MAT spreadsheet and shares biweekly with outpatient team. Maintain familiarity with community resources and collaborates with other agencies and community-based supports. Spend a minimum of 32 hours engaged in face-to-face contact with individuals who may be in need of opioid or other substance use disorder treatment, to include the homeless or those at risk of homelessness. At least 16 of the 32 hours is spent on the van in approved locations. Conduct street outreach in areas of the community where substance use, overdose, and drug transactions are known to occur in an attempt to engage with and treat individuals at risk of opioid overdose. Provide direct care, under the supervision of a licensed/credentialed clinician and in collaboration with members of the multidisciplinary team, by facilitating/co-facilitating therapy groups, educational groups and performing assigned case management activities to assist in achieving desired outcomes and to meet best practices. Identify client needs outside the scope of the agency and assists clients by making appropriate referrals, utilizing other support systems and available community resources. Develop relationships with referral sources and community partners including attendance of meetings. Participate in staff meetings and in-service trainings as required to ensure quality of care offered to clients. Participate in regularly scheduled supervision. As part of the treatment team, identifies and observes progress of recovery plan, contributes to the revision of the recovery plan and initiates interventions identified on the recovery plan. Identify clients who are experiencing barriers to accessing treatment and assist with securing entitlements and resources. Assist in Basic Needs applications and resources. Assist in engagement and retention activities for outpatient clients. Administer and/or supervises oral swab and urine toxicology screenings. Complete inventory regularly on tox screening supplies and request ordering. Respond to records requests and Quest/Cordant paperwork as needed. Perform other duties as assigned.
MINIMUM REQUIREMENTS:
High school diploma / GED required. Certificate of completion from a recognized peer/recovery coach program required. Willingness to disclose status of recovery, with a minimum of two years of recovery from Substance Use/Mental Health Disorders. Knowledge of chemical dependency treatment, principles of self-help support groups, and available community resources. Valid driver’s license and access to reliable transportation. Ability to work a flexible schedule. Able to drive the mobile MAT van. Good communication and documentation skills. Ability to use basic computer programs, including Microsoft Office and electronic health record. Maintain CPR/First Aid certification. Willingness to become trained in the administration of naloxone.
