This role provides comprehensive outpatient care, including patient assessment, diagnosis, treatment, care coordination, patient education, and ongoing clinical management. The ideal candidate is a clinically skilled, compassionate provider who thrives in a collaborative environment and is passionate about delivering exceptional patient care.</p><p><br></p><p>This position is ideal for Nurse Practitioners with experience in infusion therapy, oncology, emergency medicine, critical care, primary care, specialty care, or other outpatient clinical settings.</p><p><br></p><p><strong>Key Responsibilities</strong></p><ul><li>Perform comprehensive patient assessments and clinical evaluations</li><li>Diagnose, treat, and manage acute and chronic medical conditions within scope of practice</li><li>Provide patient and family education regarding treatment plans and infusion therapies</li><li>Monitor patients receiving specialty infusion medications and respond appropriately to adverse reactions or complications</li><li>Collaborate with physicians, nurses, pharmacists, and other healthcare professionals to ensure coordinated patient care</li><li>Review laboratory results and clinical data to support treatment decisions</li><li>Perform IV assessments and support infusion-related clinical care</li><li>Maintain accurate, timely, and compliant medical documentation</li><li>Participate in quality improvement initiatives and clinical program development</li><li>Serve as a patient advocate and help patients navigate the healthcare continuum</li><li>Support a positive patient experience through compassionate, patient-centered care</li><li>Perform other duties as assigned</li></ul><p><strong>Required Qualifications</strong></p><ul><li>Master's Degree or higher from an accredited Nurse Practitioner program</li><li>Current, unrestricted Nurse Practitioner license in Alabama</li><li>Current national certification as a Nurse Practitioner (ANCC or AANP)</li><li>Current DEA registration and prescriptive authority, or ability to obtain</li><li>Current BLS certification (hands-on/in-person certification required)</li></ul><p><strong>Experience Requirements</strong></p><ul><li>Minimum two (2) years of experience as a Nurse Practitioner in a clinical setting <strong>OR</strong></li><li>Three (3) or more years of Registered Nurse experience in Oncology, Infusion Therapy, Emergency Department, Intensive Care Unit, Specialty Care, or related clinical setting</li><li>Infusion therapy experience strongly preferred</li><li>Experience managing patients with acute and chronic medical conditions</li><li>Strong clinical assessment and decision-making skills</li></ul><p><strong>Preferred Experience</strong></p><ul><li>Outpatient infusion center experience</li><li>Oncology, Rheumatology, Neurology, Gastroenterology, Immunology, or Specialty Pharmacy experience</li><li>Vascular access and IV therapy expertise</li><li>Chronic disease management experience</li><li>Ambulatory care or outpatient practice experience</li></ul><p><strong>Knowledge, Skills &amp; Abilities</strong></p><ul><li>Excellent verbal, written, and electronic communication skills</li><li>Strong organizational and multitasking abilities</li><li>Ability to work independently and practice autonomously</li><li>Strong critical thinking and problem-solving skills</li><li>Ability to collaborate effectively within a multidisciplinary healthcare team</li><li>Adaptability and flexibility in a fast-paced healthcare environment</li><li>Commitment to providing exceptional patient-centered care</li></ul><p><strong>Why Join TwelveStone?</strong></p><ul><li>Collaborative and supportive clinical team</li><li>Predictable outpatient schedule</li><li>Opportunity to work with innovative biologic and specialty therapies</li><li>Meaningful patient relationships and continuity of care</li><li>Mission-driven organization focused on improving patient outcomes</li></ul><p>TwelveStone Health Partners is an Equal Opportunity Employer.</p>",
