The Ryan White Support Services program addresses unmet social service needs that have significantly hindered their access to and engagement in HIV and other essential medical care. This program will expand services to Staten Island and establish community-based, non-Medicaid eligible services in Queens to assess, identify, and address unmet social determinants of health (SDOH) needs.</p><p>The Community Navigator (Staten Island) will be responsible for delivering non-medical case management and food nutrition education to clients enrolled in our Ryan White Support Services program, located on the north shore of Staten Island. The Community Navigator will be trained on NYSDOH-approved HIV Health Education/Risk Reduction curriculum, food and nutrition counseling/education, and developing an individualized Healthy Food Security Plan for clients.</p><p><br></p><p><strong>DUTIES &amp; RESPONSIBILITIES:</strong></p><ul><li>Complete assessments and develop a coordinated service plan with the client, the client’s family, and the care team.&nbsp;</li><li>Document the client’s progress towards treatment and monitor client progress.</li><li>Collaborate with Health Educator and Peer Advocate to identify appropriate health education services for clients.</li><li>Manage the Healthy Food Security intervention, providing nutrition counseling/education, grocery gifts cards and development of food security plan.</li><li>Maintain updated case records through the agency’s EMR in accordance with Health Research Services Administration (HRSA) defined services, agency standards and regulatory requirements</li><li>Accompany clients to medical appointments and community agencies for support and advocacy.</li><li>Assess treatment adherence and addresses barriers as they arise.</li><li>Reassess client needs including the Self-Management Assessments and program eligibility.</li><li>Coordinate with Service Providers and provides critical feedback to members of the team.</li><li>Actively participate in case conferences, clinical supervision, and individual supervision</li><li>Meet training requirements as indicated by the funder including the 6-day Core training.</li><li>Perform all functions in alignment with CHASI’s Mission, Vision, and Core Values</li><li>Other duties as assigned.&nbsp;</li></ul>",
