Their responsibilities focus on restoring mobility, strength, and function to help patients transition to a lower level of care (e.g., home or outpatient therapy). &nbsp;The PT utilizes <strong>GG scoring</strong> to assist patient progress and document functional outcomes in compliance with CMS (Centers for Medicare &amp; Medicaid Services) and other regulatory requirements.&nbsp;</p><p><strong>POSITION&rsquo;S ESSENTIAL RESPONSIBILITIES:&nbsp;</strong></p><p><strong>1. Patient Evaluation &amp; Treatment Planning</strong></p><ul><li>Perform <strong>initial and ongoing assessments</strong> of patients&rsquo; functional mobility, strength, balance, and endurance.</li><li>Develop <strong>personalized treatment plans</strong> with measurable goals in collaboration with physicians, OTs, SLPs, social workers/case management and nursing staff.</li><li>Utilize evidence-based interventions to maximize recovery.</li></ul><p><strong>2. Rehabilitation &amp; Functional Training</strong></p><ul type=\"disc\"><li>Provide <strong>intensive therapy</strong> to improve:</li></ul><ol><li><strong>Bed mobility</strong> (rolling, sitting edge-of-bed)</li><li><strong>Transfers</strong> (bed-to-chair, toilet, car)</li><li><strong>Gait training</strong> (walking with assistive devices as needed)</li><li><strong>Stair negotiation</strong></li><li><strong>Wheelchair mobility &amp; safety</strong> (if applicable)</li></ol><ul><li>Implement <strong>therapeutic exercises</strong> for strength, flexibility, and endurance.</li><li>Use <strong>modalities</strong> (e.g., electrical stimulation, ultrasound, heat/cold therapy) as appropriate.</li></ul><p><strong>3. Patient &amp; Caregiver Education</strong></p><ul><li>Educate patients and families on&nbsp;home safety, fall prevention, and proper use of assistive devices.</li><li>Train caregivers in&nbsp;safe transfer techniques and mobility assistance.</li><li>Provide guidance on&nbsp;post-discharge exercises and adaptive strategies.</li></ul><p><strong>4. Interdisciplinary Collaboration</strong></p><ul><li>Participate in <strong>weekly team meetings</strong> to discuss patient progress and discharge planning.</li><li>Communicate effectively with physicians, nurses, case managers, and other therapists.</li><li>Adjust treatment plans based on patient response and medical status.</li></ul><p><strong>5. Documentation &amp; Compliance</strong></p><ul><li>Accurately complete <strong>Section GG functional scoring</strong> at admission, discharge, and per IRF-PAI guidelines</li><li>Maintain <strong>accurate and timely documentation</strong> in electronic medical records (EMR).</li><li>Ensure compliance with <strong>Medicare, insurance, and facility guidelines</strong>.</li><li>Complete progress notes, discharge summaries, and equipment justification reports.</li><li>Follow hospital protocols (e.g. isolation precautions, code responses, etc)</li></ul><p><strong>6.
