Responsibilities Supports physicians in Division of Cardiovascular Electrophysiology Medicine in providing patient care that is safe, effective, patient-centered, timely, efficient and equitable. Organizes and makes decisions to gather medical information for the division in order to facilitate the provider’s assessment of the patient in order to develop and implement an individualized patient plan of care. Ensures that the patient’s medical needs are addressed; consult with the patient’s physicians as needed, coordinating plans of treatment, and advocating for the patient when necessary. Fosters intra-facility and inter-facility working relationships to help accomplish goals. Acts as a liaison between the division, clinic nursing staff, device clinic, device reps, non-invasive lab, EP lab, ACC. Obtains and organizes medical information from OSUP system as well as outside facilities, including obtaining records/information as needed from the PCP, cardiologist, pharmacy, and directly from patient Coordinates patient care diagnostic testing and/or interventional procedures, which has been ordered by physicians. This may include communications with the Wexner Medical Center and with other multiple outreach facilities. Serves as the patient facilitator and educator for potential and/or confirmed EP patient conditions/diagnoses, treatments, and procedures. Performs and coordinates initial assessments and ongoing reassessments of the patient’s status; documents patient case information within the electronic medical record (EMR); performs chart review/audit. Evaluates patient to gather basic history, accurate medication list and other clinical information. Observe patients and reporting changes in patients' conditions, such as adverse reactions to medication . Measure and record patients' vital signs, such as height, weight, temperature, blood pressure, pulse and respiration. Help maintain patient flow and workflow. Work collaboratively as a team member of the clinic staff to provide a high quality and efficient service to patients and clinic. Recognizes patient rights and provides for confidentiality of patient information and for dignity through own performance and in relationship with others. Collaborates with other disciplines; promotes and supports research activities; and utilizes available resources to provide and improve patient care. Working within scope of license, develop and implement nursing plan of care for patients. Communicate the plan of care to patients as prescribed by physician: test results, scheduling of specialized tests and treatment modalities, prescription drug instructions and other patient education including procedures and/or disease processes. Participates in quality improvement team efforts, including meetings to discuss quality assurance improvement initiatives. Participate in special projects as needed including but not limited to development of nursing documentation forms, development or procurement of patient education forms, quality assurance activities, etc. Role may include assisting the provider with real-time documentation of each patient’s medical chart during his or her appointment. Travel required to support clinics and electrophysiology providers within our metro Columbus area locations.Travel required to support clinics and electrophysiology providers outside of the Columbus Metro area to our outreach locations and/or affiliations including but not limited to Lancaster, Portsmouth, Cambridge, Bellefontaine, Marysville, and Newark.Travel may be required to accommodate staffing levels at other IM clinical facilities Performs other related duties as required, and other duties as assigned. Qualifications Associate Degree in Nursing from a CCNE (Commission on Collegiate Nursing Education) or Accreditation Commission for Education in Nursing (ACEN) accredited nursing program. Current Ohio Registered Nurse license. CPR/BLS certified. ACLS certified required 2 years of Nursing experience Knowledge of triage procedures along with clinic, physician office and/or urgent care experience. Ability to maintain quality, safety, and/or infection control standards. Knowledge of appropriate procedures and standards for the administration of medications and patient care aids. Ability to educate and clearly communicate with patients and/or families as to the nature of disease and to provide instruction on proper care and treatment. Ability to convey information both in writing and verbally. High level of interpersonal skills to handle sensitive and confidential situations. Position continually requires demonstrated poise, tact and diplomacy. Analytical ability and critical thinking to gather and interpret data and develop, recommend, and implement solutions.
