Description Under general supervision of the Chief Nursing Officer, Associate Chief Nursing Officer and direct supervision RN-1, Nurse Manager or Charge Nurse, uses the nursing process to safely coordinate total patient care and education for an assigned number of patients. At all times, patient assignments will be determined by adjustment of department patient census and current staffing levels for the overall patient care which are reviewed shift-to-shift. As determined by the Nurse Manager, the nurse will be assigned up to 40 hours per week and may rotate between day and night shift according to the needs of the unit and staff. The nurse is expected to function within the scope of approved policies, procedures, and regulations for the department and organization. Will be responsible to assist with orientation, preceptorship, and management of personnel assigned to unit. Must be able to work and relate in a professional, non-defensive manner with peers, providers, mid-level providers, administration, patients, and visitors. Will be responsible to document total care given, review and maintain patient medical record per policy, and assist with accurate and timely entry of patient data, orders, and charges into computer system. Nurse demonstrates responsibility for the assessing, monitoring and management of patients developing and initiating a plan of care, administering medication and treatments as prescribed, collaborating with teams for patient care, ordering, evaluating and performing diagnostic test ( i.e. stool, sputum, urine), review and interpret result of test performed, conduct physical examination head to toe assessment as the beginning of their visit to assess the overall health of their patients, monitor patients health record any relevant symptoms, carefully checking patients record to ensure the correction mediation, dosages are listed, maintain IV lines to ensure they are changed regularly, monitoring patients vital signs, assessing and recording nonverbal cues, provide support, education and advise to patients to make sure they feel cared for, listened and provided comfort. The nurse will demonstrate knowledge and conduct physical exam head to toe assessments, record detailed medical history and symptoms, demonstrate knowledge of operating medical equipment (i.e. stethoscopes, glucometers, pulse oximeters, thermometer, blood pressure machines, IV pumps, code carts, wound drainage systems, ventilation equipment, etc.). The nurse will demonstration knowledge to education patient about their illness, treatments and medications, providing clear instructions, on how they can manage their symptoms ( i.e. explain meds what type med, why they are taking med, side effects of meds, importance of follow up appointment, rehab, things to do at home to improve and prevent readmission). The nurse must ensure the safety of their patients, nurse advocates for their patients by taking the time to actively listen to their concerns, encouraging patient to ask questions, respecting their wishes and communicate what the patients wants with family and other team members and provides emotional support to both patient and family. Nurses are responsible to ensure basic care needs are completed and/or assist with basic needs as indicated (bathes, turning, cleaning, feeding, hygiene related activities, beside nurse provide emotional support, administer medications and track vitals. The nurse is to assist with ordering supplies/ inventory. The nurse reviews, records and reports irregularities/problems. Nurse regularly educates providing clear instructions and teaches patient and family about the illness, teaches how to manage illness (taking meds as prescribed, importance of follow up appointments, rehab etc.) procedures, and symptoms as well as the plan of care upon leaving. Maintains clinical competence, continually learning to perform any job duty within the scope of RN practice. Maintains knowledge about and ensures delivery of nursing care at or above standards of practice for nursing. Administers medication safely. Demonstrates knowledge of and administers all medications according to provider’s order and hospital policy adhering to the 6 rights of medication administration: (i.e. Right Patient, Right Drug, Right Dose, Right Time (be aware of dosing frequency, schedule/med log, set an alarm, clearly communicate), Right Route, Right Documentation), Demonstrates know of high risk and critical medications (i.e. Insulin, potassium K-riders, vancomycin Peak / trough, heparin, Coumadin, knowledge of timeframe for lab drawn for indicated meds i.e. PT, PTT etc.). Documents adverse drug reactions and reports per departmental and pharmacy policy. Performs IV therapy while demonstrating skill and accuracy per provider’s order and hospital policy. Monitors and documents the patient’s level of consciousness, demonstrates knowledge and performs wound care according to hospital protocol/policy. Promotes unit and organizational philosophy and goals; involves staff in the development of unit goals. Supports and assists with staffing/scheduling policies to meet unit needs. Assists with establishment of departmental policies and procedures that reflect the services provided by the department. Participates in organizational committees as assigned. Provides important information on trends related to patient outcomes to Nurse Manager. Maintains current knowledge of regulatory requirements. Demonstrates professionalism and reinforces professional behavior of staff members. Maintains current knowledge of regulatory requirements: IFC, CDC, DNV, MSDH, MSBN, Baby Friendly, Quality initiatives and CMS. Demonstrates knowledge and skill related to patient throughput and customer service / satisfaction providing kind, compassionate care to all patient types. Nurses are required to float between departments as indicated by units needs and census, staffing is reviewed by mangers and RN 1 shift to shift - this is non-negotiable and it’s an expectation. WE are all on the same team, teamwork is expectation to complete all task for patient care needs- Our patients are ALL our patients (i.e. there is no ME in TEAM). Pays close attention to the census with special emphasis on patient throughput. Addresses any patient concerns at the bedside, report concerns to charge nurse or RN-1 as indicated. The successful candidate will have to pass a pharmacology test for this position. Complete the yearly mandatory in-service (YMI) requirements is nonnegotiable and will be the responsibility of the employee to arrange, attend sessions and maintain current on Healthstream.
