Position Summary: To provide case management services for a caseload of clients with severe mental illness who are homeless, at risk of homelessness or transition to independent living in the Mississauga/South Etobicoke area. Case Managers work in partnership with clients to develop the skills and supports they need to live successfully in the community. Clients are supported through visits in their own home, in the community or in the office.
