Position Summary:
To provide case management services for a caseload of clients with severe mental illness who are homeless, at risk of homelessness or transition to independent living in the Mississauga/South Etobicoke area. Case Managers work in partnership with clients to develop the skills and supports they need to live successfully in the community. Clients are supported through visits in their own home, in the community or in the office.
Responsibilities:
Manage a full caseload and provide case management support to assigned individuals Complete initial assessment outlining individual psychosocial needs and determining eligible for the program Engage individuals in the Recovery process through a goal oriented, skills based approach Work in partnership with the team and the partnering housing provider to support successful transition to independent living Ongoing monitoring of individual's wellness, assessment of risk and overall progress in one's Recover Process Liaise with existing treatment team including physicians
Qualifications:
Professional qualification in a health discipline Over 5 years' experience with adult clients with serious mental illness Case management experience with mental health clients Experience working with people who are homeless or have frequent contact with shelters an asset Experience working in addictions an asset Training and working knowledge in the area of Recovery Proficient computer skills Working knowledge of community resources and the Residential Tenancies Act (RTA) Excellent communication, organization, presentation and problem solving skills Treatment planning and intervention experience required Valid driver's license, relevant insurance and use of own vehicle Demonstrated ability to work independently and as a part of a team Ability to speak a second language would be a definite asset Proven skills in the area of Crisis Intervention, Suicide Risk Assessment, and Mental Status Assessment
