Evenings and weekends will be required. </span></p>\n<p><span> </span></p>\n<p><span>Compensation:</span><span> $18.50/ hour</span></p>\n<p><br></p>\n<p><br></p>\n<p><span style=\"font-weight: bold\"><span style=\"font-size: 15px\">Main Job Tasks and Responsibilities</span></span></p>\n<ul>\n<li><span style=\"font-size: 15px\">Consistently exercises individual judgment, makes decisions and solves problems.</span></li>\n</ul>\n<ul>\n<li><span style=\"font-size: 15px\">Create monthly progress reports to submit to DVR that include services provided and challenges the individual may face. </span></li>\n<li><span style=\"font-size: 15px\">Completes daily notes of individual’s progress and challenges, timesheet and goal tracking in OnTarget.</span></li>\n<li><span style=\"font-size: 15px\">Works with Program Director to update and maintain program participant records.</span></li>\n<li><span style=\"font-size: 15px\">Reports complaints or performance concerns; manages conflicts and disagreements in a constructive manner; responds to grievances; identifies and solves individual’s problems.</span></li>\n<li><span style=\"font-size: 15px\">Represent Program Director, Director or designee at internal and external meetings.</span></li>\n<li><span style=\"font-size: 15px\">Coaches real-life job skills to individuals with disabilities at their workplace while providing the needed supports to help them obtain and maintain meaningful employment in the community.</span></li>\n<li><span style=\"font-size: 15px\">Ongoing communication with workplace supervisors, team members, and family members.</span></li>\n<li><span style=\"font-size: 15px\">Provides on-site training, instruction, supervision, and advocacy for individuals at community job sites.</span></li>\n<li><span style=\"font-size: 15px\">Maintains and distributes employment information such as job openings to Program Director and individuals.</span></li>\n<li><span style=\"font-size: 15px\">Support individuals in increasing independence in their workplace and community.</span></li>\n<li><span style=\"font-size: 15px\">Assist individuals in understanding the functions and additional tasks of their job.</span></li>\n<li><span style=\"font-size: 15px\">Assist individuals in communicating and cultivating meaningful relationships with coworkers and management within their job and professional community.</span></li>\n<li><span style=\"font-size: 15px\">Assist individuals in advocating for their needs while at work, including potential formal accommodations needed. </span></li>\n<li><span style=\"font-size: 15px\">Maintain proper community supervision requirements.</span></li>\n<li><span style=\"font-size: 15px\">Transportation to and from individual's work/interviews, if required.</span></li>\n<li><span style=\"font-size: 15px\">Provides medication administration to individuals served when needed under QMAP certification.</span></li>\n<li><span style=\"font-size: 15px\">Proactively performs outreach to cultivate relationships with perspective employers. </span></li>\n<li><span style=\"font-size: 15px\">Maintains a healthy relationship with individuals serving as their single point of contact as needed; checks in regularly with the individual throughout their employment and connects them to any supports or resources needed through job placement.</span></li>\n<li><span style=\"font-size: 15px\">Carries out duties as assigned by Program Director and/or designated supervisor.</span></li>\n<li><span style=\"font-size: 15px\">Performs all other duties as assigned.</span></li>\n</ul>\n<p><br></p>\n<p><span>Qualifications:</span></p>\n<ul>\n<li><span>Must have reliable transportation, a valid driver's license, and a willingness to transport clients in your vehicle</span></li>\n<li><span>Personal vehicle use</span></li>\n<li><span>Must be at least 18 years of age due to driving demands</span></li>\n<li><span>Must be able to work evenings and weekend</span></li>\n<li><span>Able to perform the physical and mental demands of the position</span></li>\n</ul>\n<p><br><br></p>\n<p><span>This is a driving position that will require the use of your personal vehicle to transport the clients we serve. The State of Colorado requires that any vehicle used for this purpose pass a detailed multi-point safety inspection through an ASE certified mechanic.  Some of the items on this inspection include windshield, suspension, brakes, lights, seatbelts and all other safety features.  If offered this position, Stellar Care will cover the cost of the inspection, however any repairs required are at the expense of the applicant and must be resolved within 30 days of hire.
