Ennoble Care is seeking a full-time Care Coordinator to join our Waco, Texas Home Office team. This on-site/in-office role serves as a key member of our interdisciplinary care team, supporting patients through proactive care coordination, collaboration, and a patient-centered approach. This Care Coordinator works closely alongside fellow Care Coordinators, providers, patients, families, and clinical staff to help ensure seamless continuity of care. Responsibilities include managing patient communication, coordinating provider schedules and visits, facilitating follow-up care, addressing barriers to care, and maintaining accurate and timely documentation within the EMR. Qualified candidates must possess a minimum of a Medical Assistant (MA) certification with at least one-year of clinical experience, and be compassionate, detail-oriented professionals who thrive in a fast-paced environment. Candidates should demonstrate strong organizational skills, adaptability, and clear, professional communication.
