This role serves as a primary operational liaison between clients, families, caregivers, internal teams, and payer requirements.&#xa0;</p><p><strong>Client Care Coordinator Requirements:&#xa0;</strong></p><p><strong>EDUCATION AND EXPERIENCE</strong></p><ul><li>High school diploma or equivalent&#xa0;</li><li>1–2&#xa0;years experience&#xa0;in home care, healthcare coordination, or client services&#xa0;</li><li>Strong organizational and communication skills&#xa0;</li><li>Ability to manage multiple priorities in a fast-paced environment&#xa0;</li><li>Experience with&#xa0;AlayaCare,&#xa0;CalAIM, or Regional Center programs preferred&#xa0;</li></ul><p><strong>Essential Functions:</strong></p><ul><li>Serve as a primary point of contact for&#xa0;scheduling&#xa0;clients and families&#xa0;</li><li>Support client&#xa0;care continuity following start of care&#xa0;</li><li>Ensure&#xa0;accurate&#xa0;collection and entry of client information in&#xa0;AlayaCare&#xa0;</li><li>Coordinate caregiver placement, scheduling, and coverage needs&#xa0;</li><li>Monitor ongoing client satisfaction and address service issues&#xa0;</li><li>Maintain&#xa0;accurate&#xa0;documentation, authorizations, and compliance requirements&#xa0;</li><li>Collaborate with Intake, Scheduling, Billing, and Operations teams&#xa0;</li></ul>",
