The Nurse Practitioner will be responsible for assessment, planning, implementation and evaluation of care for clients in the community both independently and collaboratively with a physician, depending upon the complexity of client needs.&nbsp; This includes: health promotion and protection; first line treatment and emergency&nbsp;services; provision of supportive counselling and instruction in management of medical conditions; disease and injury prevention; initiating and conducting community needs&nbsp;assessments and participating in evaluative projects; and collaboration with and referral to other health care professionals.&nbsp; This is a community-based position therefore the incumbent is required to work at a high level of independent clinical practice.</p>\n<p>This position will be part of the Intermediate Care Service within the Primary Health Care Networks. This service addresses a provincial health system service gap, where some seniors&nbsp;(adults aged 65+) with multiple complex health issues have difficulty in accessing an office based primary care provider. The goals for this service are: to provide the most at risk&nbsp;seniors with home based access to higher level health care providers, including MDs and NPs; to mitigate the risk for worsening health states and acute care episodes leading to&nbsp;ED visits and hospitalizations for at risk seniors; to reduce system pressures on acute care services, particularly EDs and hospitals; to provide cost effective and high quality health&nbsp;care using a patient centered and community based approach.
