    "ExternalDescriptionStr": "<div><div><div><div><div><p>FlexStaff is hiring a Clinical Assessment Manager - Care Delivery (UAS Nurse) to join our client's team on a temp to perm basis.&nbsp;</p><p>Location: Brooklyn (Orientation in the Bronx)<br>Setting: Field; conducting in-home participant assessments.<br>Pay Rate: $57/hr<br>Schedule: Monday - Friday 8:30am-5:30pm, (1 hour lunch) (FT)<br>&nbsp;</p><p class=\"\">We are seeking a <strong class=\"\">Clinical Assessment Manager</strong> with required <strong class=\"\">UAS experience</strong> to oversee and execute all tasks related to the clinical assessment process. This includes, but is not limited to, clinical onboarding, reassessment, UAS-NY completion, comprehensive assessment, medication review, and post-hospital discharge evaluation. The role involves close collaboration with intake, enrollment, and care delivery teams, conducting in-home assessments, ensuring accurate documentation in EMR, promoting the PACE program, and maintaining compliance with CMS/PACE guidelines. This individual will play a crucial part in facilitating smooth transitions for participants, advising on operational improvements, and actively contributing to quality improvement initiatives.</p><p class=\"\">Here are the key responsibilities for this role:</p><p class=\"\"><strong class=\"\">Assessment &amp; Eligibility:</strong></p><ul class=\"\"><li class=\"\">Conduct in-home or virtual assessments using the Uniform Assessment System of New York (UAS-NY) for prospective or enrolled participants.</li><li class=\"\">Determine initial or continued plan eligibility in accordance with Community Based Long Term Care (CBLTC) Services requirements.</li><li class=\"\">Answer questions about the PACE program and highlight its benefits during assessments.</li><li class=\"\"><p>Maintain a schedule of at minimum 2-3 UAS-NY assessments per day.</p><p>&nbsp;</p><p><strong class=\"\">Documentation &amp; Compliance:</strong></p><ul class=\"\"><li class=\"\">Evaluate and review previous assessments, primary diagnoses, and medications, ensuring all changes are accurately reflected in the EMR and match UAS entries.</li><li class=\"\">Document all interactions, notes, service plans, and other documentation within 48 hours.</li><li class=\"\">Work with the quality review team to ensure accuracy and completeness of documentation in both the Health Commerce System and the EMR.</li><li class=\"\"><p>Maintain knowledge of all applicable CMS/PACE guidelines and organizational policies.</p><p>&nbsp;</p></li></ul><p><strong class=\"\">Collaboration &amp; Communication:</strong></p><ul class=\"\"><li class=\"\">Collaborate closely with intake, enrollment, and care delivery teams.</li><li class=\"\">Escalate significant changes or concerns during assessments to the quality review team or discipline manager.</li><li class=\"\">Provide significant clinical information to the Care Delivery team in advance of enrollment for smooth transitions.</li><li class=\"\">Partner with Care Delivery leadership and the Interdisciplinary Team (IDT) to provide updates, address concerns, and improve processes.</li><li class=\"\">Serve as the clinical point person for Care Delivery questions post-enrollment.</li><li class=\"\"><p>Remain responsive and available by phone/email for CenterLight staff.</p><p>&nbsp;</p><p><strong class=\"\">Program Promotion &amp; Education:</strong></p><ul class=\"\"><li class=\"\">Promote and educate potential referral sources about the PACE program.</li><li class=\"\">Educate potential/new PACE participants on plan benefits, services, and the care delivery model.</li></ul></li></ul><p><strong class=\"\">Process Improvement &amp; Quality:</strong></p><ul class=\"\"><li class=\"\">Complete and/or reassess PCAT Tasking Tool, providing recommendations for modifications.</li><li class=\"\">Utilize the Lenavi analyzer during review and assessment processes.</li><li class=\"\">Engage in continuous quality improvement activities to promote optimal staff performance and participant outcomes.</li><li class=\"\">Advise on operational process improvements related to enrollment, scheduling, financials, reporting, and engagement strategies.</li><li class=\"\">Regularly review key performance metrics and analytics.</li></ul><p><strong class=\"\">Logistics &amp; Professionalism:</strong></p><ul class=\"\"><li class=\"\">Must be able to work some weekends and travel to various boroughs.</li><li class=\"\">Act only within the scope of individual's authority to practice.</li><li class=\"\">Meet a standardized set of competencies for the position.</li><li class=\"\">Serve as an acting member of the IDT.</li></ul><p style=\"font-family: Calibri; font-size: 11pt; margin: 0in;\"><strong>Additional Salary</strong></p><p style=\"font-family: Calibri; font-size: 11pt; margin: 0in;\">&nbsp;</p><p style=\"font-family: Calibri; font-size: 11pt; margin: 0in;\"><strong>&nbsp;Detail</strong></p><p style=\"font-family: Calibri; font-size: 11pt; margin: 0in;\"><strong>The salary range and/or hourly rate listed is a good faith determination of potential base compensation that may be offered to a successful applicant for this position at the time of this job advertisement and may be modified in the future.
