Description

Position Summary The Implementation Accompaniment Lead (Prosperemos Juntos | Thriving Together - PJTT) accompanies community coalitions in shifting power and addressing root causes of health inequities by applying PJTT frameworks to real-world challenges. This role requires strong independent judgment, relational awareness, and the ability to translate frameworks into action across varied community contexts. Serving as a program officer, subject matter expert, and coalition coach, the Implementation Accompaniment Lead facilitates conversations that may include tension or competing priorities while maintaining alignment with PJTT principles. Success in this role requires adaptability, confidence in evolving environments, and the ability to move from learning to independent implementation over time. Salary The annual salary for this position starts at $62,339.00, with a midpoint of $79,482.00. Most new hires begin within this range, based on their years of directly related experience and education. Candidates with exceptional qualifications that exceed the minimum requirements may be considered for compensation above the midpoint, in alignment with internal equity and IPM (Individual Pay Model) compensation guidelines. Scope and Impact The role influences community coalitions across MHM’s service area, guiding them in building equitable strategies that impact health outcomes and community well-being. This position has a regional impact, requiring up to 50% travel, and contributes to MHM’s broader health equity initiatives by strengthening coalition governance, cross-sector partnerships, and community engagement. Decision-Making Authority This role requires strong independent judgment in applying PJTT frameworks across varied community contexts. The Implementation Accompaniment Lead is expected to make thoughtful, real-time decisions while facilitating coalition learning, navigating tension or competing priorities, and maintaining alignment with PJTT values. Autonomy is expected in tailoring approaches to community strengths, needs, and emerging challenges. Interactions / Working Relationships Frequent collaboration with PJTT and Communities of Solutions (CoS) team members. Regular engagement with community coalitions, Community Connectors, persons with lived experience, faith communities, schools, nonprofits, and other external stakeholders. Partnership with internal MHM departments to integrate health equity practices and advance community-driven initiatives. Essential Duties and Responsibilities Manage trust-based philanthropic relationships with coalitions by developing trust, providing guidance, monitoring performance & impact, and engaging MHM colleagues. Reteach and reinforce use of PJTT frameworks (e.g., Pathways to Population Health, Vital Conditions for Community Well-Being, stakeholder mapping, driver diagrams, 90-day action planning) in public and community settings. Serve as a PJTT subject matter expert advising coalitions on governance, power shifting, and conflict management. Provide ongoing (at least once a month) guidance and encouragement to coalitions to support community-driven health equity strategies. Develop and deploy capacity-building opportunities, including trainings, toolkits, leadership development, and webinars. Participate in the planning, implementation, and continuous improvement of PJTT events. Facilitate inclusive community conversations to share and collect information that informs program planning and evaluation. Build and maintain strong relationships with community-based organizations, faith communities, and schools to ensure communication and engagement. Collect, analyze, and report coalition progress using qualitative and quantitative data. Participate in PJTT curriculum development and tactical planning. Assess Learning Phase coalitions for readiness to enter the PJTT Implementation Phase through use and improvement of defined Implementation Phase rubric. Prepare coalitions to enter the PJTT Sustainability Phase through continually assessing their readiness. Represent MHM in community meetings and task forces. Perform other duties as assigned. (Approximate % of role: Coalition coaching/support 40%; Presentations & facilitation 25%; Capacity-building 20%; Reporting/other duties 15%) Supervisory or Leadership Responsibilities This position does not supervise others but provides coaching, facilitation, and subject matter expertise to coalitions, Community Connectors, and cross-sector partners. Qualifications Minimum Qualifications (Required) Education: Bachelor’s degree in social/behavioral sciences, public health, or a related field. Three (3) years of experience in community outreach, organizing, or coalition building, including direct work with individuals who have lived experiences of inequities. In lieu of a degree: High school diploma or equivalent and five (5) years of progressively responsible experience in community outreach, organizing, or coalition-building roles. Licenses/Certifications: None Experience: Demonstrated experience engaging individuals with lived experience in projects, programs, and/or decision-making processes to shift power to community members. Experience and knowledge in conflict resolution. Preferred Qualifications Knowledge of community development models, such as Asset-Based Community Development (ABCD). Experience addressing health and social inequities in marginalized populations. Experience in adult education and / or learning & development of curriculum. Background in community organizing and working with grassroots organizations, including partnerships with faith-based organizations. Strong project management, public speaking, facilitation, and relationship-building skills. Ability to work across departments and manage multiple priorities. Knowledge, Skills, and Abilities (KSAs) Collaboration, communication, conflict resolution, and problem-solving. Strategic and analytical thinking with a learning-oriented, “fail forward” mindset. Strong facilitation skills to engage diverse community voices. Relationship-building across sectors with cultural competence and adaptability. Language Skills Required: Fluency in English (written and spoken). Preferred: Bilingual in Spanish, with ability to present and respond to diverse audiences, develop documents, and facilitate bilingual discussions. Team members who demonstrate fluency in both Spanish and English will receive additional pay and be expected to translate small amounts of written text from English to Spanish, especially when working under time constraints (not to exceed 5% of the team member’s total weekly time) Technology and Tools Microsoft Office Suite (Outlook, Word, Excel, PowerPoint).
