impact, and engaging MHM colleagues.</li><li style=\"margin-left:0in;\">Reteach and reinforce use of PJTT frameworks (e.g., Pathways to Population Health, Vital Conditions for Community Well-Being, stakeholder mapping, driver diagrams, 90-day action planning) in public and community settings.</li><li style=\"margin-left:0in;\">Serve as a PJTT subject matter expert advising coalitions on governance, power shifting, and conflict management.</li><li style=\"margin-left:0in;\">Provide ongoing (at least once a month) guidance and encouragement to coalitions to support community-driven health equity strategies.</li><li style=\"margin-left:0in;\">Develop and deploy capacity-building opportunities, including trainings, toolkits, leadership development, and webinars.</li><li style=\"margin-left:0in;\">Participate in the planning, implementation, and continuous improvement of PJTT events.&nbsp;</li><li style=\"margin-left:0in;\">Facilitate inclusive community conversations to share and collect information that informs program planning and evaluation.</li><li style=\"margin-left:0in;\">Build and maintain strong relationships with community-based organizations, faith communities, and schools to ensure communication and engagement.</li><li style=\"margin-left:0in;\">Collect, analyze, and report coalition progress using qualitative and quantitative data.</li><li style=\"margin-left:0in;\">Participate in PJTT curriculum development and tactical planning.</li><li style=\"margin-left:0in;\">Assess Learning Phase coalitions for readiness to enter the PJTT Implementation Phase through use and improvement of defined Implementation Phase rubric.&nbsp;</li><li style=\"margin-left:0in;\">Prepare coalitions to enter the PJTT Sustainability Phase through continually assessing their readiness.&nbsp;</li><li style=\"margin-left:0in;\">Represent MHM in community meetings and task forces.</li><li style=\"margin-left:0in;\">Perform other duties as assigned.</li></ul></div><p style=\"margin-left:0in;\"><em>(Approximate % of role: Coalition coaching/support 40%;
