This team provides hospice services to patients in their homes throughout southern Rhode Island.&nbsp;HopeHealth is a fast-growth and innovative not-for profit healthcare organization, focused on caring for people with serious illness. We deliver a unique array of medical care, care management and support services throughout Rhode Island.</span></p><p><span style=\"color: rgb(0, 0, 0);\"><span style=\"font-size: 14px; font-family: arial, sans-serif;\">This is a part-time, 32-hours a week position, with 8:00-4:30 hours and a rotating weekend commitment up to every other weekend.</span></span></p><p>This position is eligible for benefits</p><p><span style=\"font-family: arial, sans-serif; font-size: 14px;\"><br></span></p><p><span style=\"color: rgb(0, 0, 0);\"><span style=\"font-size: 14px; font-family: arial, sans-serif;\"><strong>Duties / Responsibilities:</strong></span></span></p><ul><li style=\"font-family: arial, sans-serif; color: rgb(0, 0, 0); font-size: 14px;\">Conducts assessments for home and nursing home patients. Assesses patient and family status based on the physiological, psychosocial, spiritual, and emotional needs presented.</li><li style=\"font-family: arial, sans-serif; color: rgb(0, 0, 0); font-size: 14px;\">Develops a written plan of care with IDT, defining goals and nursing actions based on needs identified in assessment of patient and family.</li><li style=\"font-family: arial, sans-serif; color: rgb(0, 0, 0); font-size: 14px;\">Implements the plan of care under the direction of the attending physician.</li><li style=\"font-family: arial, sans-serif; color: rgb(0, 0, 0); font-size: 14px;\">Evaluates the plan of care on a regular basis and as necessary to meet the changing needs of the Hospice patient and caregiver.</li><li style=\"font-family: arial, sans-serif; color: rgb(0, 0, 0); font-size: 14px;\">Instructs the caregiver in the care of the terminally ill based on the individual personal differences, educational background and previous experience.</li><li style=\"font-family: arial, sans-serif; color: rgb(0, 0, 0); font-size: 14px;\">Demonstrates skill in interpersonal relationships with Hospice patient, families, nursing home staff, co-workers and community support services.</li><li style=\"font-family: arial, sans-serif; color: rgb(0, 0, 0); font-size: 14px;\">Maintains accurate records in designated manner to record observations, problems and nursing actions which reflect the care given to Hospice patients / families at time of visits.</li></ul><p><span style=\"color: rgb(0, 0, 0);\"><span style=\"font-size: 14px; font-family: arial, sans-serif;\">&nbsp;</span></span></p><p><span style=\"color: rgb(0, 0, 0);\"><span style=\"font-size: 14px; font-family: arial, sans-serif;\"><strong>Education / Experience:</strong></span></span></p><ul><li style=\"font-family: arial, sans-serif; color: rgb(0, 0, 0); font-size: 14px;\">Two years hospice or oncology / homecare nursing experience preferred.
