    "description": "<h2>Job Description</h2> \n<strong>CARE AND BE CARED FOR – THIS IS YOUR HOME</strong>\n<p>As a <strong>Bilingual Social Worker (MSW)</strong>, you will assess and determine patient care needs and eligibility, provide access and referrals to community services, and engage with patients, caregivers and other health care practitioners. </p>\n<p> </p>\n<p>Whether you work in our office, in the community, or a health care facility – you will play a lead role in providing connected, accessible, patient-centered care – and be supported by our collaborative team that includes over 9,000 regulated health care and other professionals.</p>\n<p> </p>\n<p>As a valued team member, your mission will be to help our patients be healthier at home, while you benefit from our supports for professional growth, personal wellness and work-life balance.</p>\n<p> </p>\n<p><strong>What will you do?</strong></p>\n<p>The Social Worker identifies the need for the delivery of Social Work services, recommends and participates in the development of programs to meet these needs and provides consultation and social work services to selected patients.</p>\n<p> </p>\n<p><strong>Primary Responsibilities:</strong></p>\n<ul>\n <li>Provides social work intervention to referred patients and families, including assessment of psycho-social needs. Refers patients and families to community agencies as appropriate.</li>\n <li>Establishes treatment goals with respect to identified problems and develops methods of achieving these goals in consultation with Care Coordinators.</li>\n <li>Ensures that patient files are initiated, maintained and updated and maintains treatment records in accordance with established guidelines and best practices.</li>\n <li>Ensures that recognition and response to identified social needs by recommending and participating in the development of guidelines and developing resource material.</li>\n <li>Establishes and maintains liaison with other health and social service providers.</li>\n <li>Participates in committees as approved.</li>\n <li>Interprets the role and objectives of the LHIN Social Work Program to the community and interested agencies.Partakes in professional development activities and maintains knowledge and ability in the field of social work.</li>\n <li>Identifies gaps in community social services that affects the psycho-social functioning of the patient and family and participates in improving the services offered</li>\n</ul>\n<p><strong>Team/Department:</strong> Stroke Rehab</p>\n<p><strong>Hours of Work:</strong> Monday to Friday, 8:30AM - 4:30PM</p>\n<p><strong>FTE:</strong> 1.0</p>\n<p><strong>Length of Temporary Asignment:</strong> August 27th, 2027</p>\n<p><strong>Starting Salary:</strong> $47.259/hr to $50.689/hr</p>\n<p><strong>Affiliation: </strong>ONA</p>\n<p><strong>Reporting To:</strong>  Manager, Patient Services </p>\n<p><strong>Office Location:</strong> Labelle</p>\n<p> </p>\n<p><strong>What must you have? </strong></p>\n<ul>\n <li>Master’s degree in Social Work and current certificate of registration with the Ontario College of Social Workers and Social Service Workers</li>\n <li>Related professional experience which would normally be attained over a period of two years in a similar Social Worker role</li>\n <li>Related experience in a similar community-based setting is an asset</li>\n <li>Experience providing service to patients in the area of stroke rehabilitation is an asset</li>\n <li>We have a mandatory COVID-19 vaccination policy.
