This position plays a vital role in supporting the person served with daily health and personal care needs while encouraging independence, comfort, safety, and community involvement.</p>\n<p><br></p>\n<p>The ideal candidate will enjoy building meaningful relationships and assisting someone who loves coloring, shopping trips, and relaxing outdoors to enjoy the weather.</p>\n<p><br></p>\n<p><span style=\"font-size: 18pt\">Essential Responsibilities</span></p>\n<p><span style=\"font-size: 14pt\">Personal Care &amp; Daily Living Support</span></p>\n<ul>\n<li>Assist with activities of daily living (ADLs), including bathing, grooming, dressing, toileting, and mobility support</li>\n<li>Maintain a clean, safe, and organized home environment</li>\n</ul>\n<p><br></p>\n<p><span style=\"font-size: 14pt\">Medical &amp; Health Support</span></p>\n<ul>\n<li>Administer medications according to physician orders and company policies</li>\n<li>Provide tube feeding care and monitoring</li>\n<li>Observe and document changes in health status, behaviors, or medical concerns</li>\n<li>Monitor for signs of illness, discomfort, or medical emergencies</li>\n<li>Communicate effectively with healthcare providers, guardians, nursing staff, and supervisors</li>\n</ul>\n<p><br></p>\n<p><span style=\"font-size: 14pt\">Community &amp; Recreational Support</span></p>\n<ul>\n<li>Accompany and support the person served during shopping outings and community activities</li>\n<li>Encourage participation in preferred hobbies and activities such as coloring and spending time outdoors</li>\n<li>Promote independence, choice-making, and quality of life</li>\n</ul>\n<p><br></p>\n<p><span style=\"font-size: 14pt\">Documentation &amp; Compliance</span></p>\n<ul>\n<li>Complete accurate daily documentation and charting</li>\n<li>Follow individualized support plans, medical protocols, and MN 245D policies/procedures</li>\n<li>Maintain confidentiality and professional boundaries at all times</li>\n<li>Attend required trainings and staff meetings</li>\n</ul>\n<p><br></p>\n<p><span style=\"font-size: 18pt\">Qualifications</span></p>\n<ul>\n<li>Current CNA certification or active LPN license</li>\n<li>Valid driver’s license and acceptable driving record</li>\n<li>Ability to pass background study requirements</li>\n<li>Experience working with individuals requiring high levels of medical and personal care preferred</li>\n<li>Experience with tube feeding and medication administration preferred</li>\n<li>Compassionate, reliable, patient, and self-motivated</li>\n</ul>\n<p><br></p>\n<p><span style=\"font-size: 18pt\">Physical Requirements</span></p>\n<ul>\n<li>Ability to stand, walk, bend, and assist throughout shifts</li>\n<li>Ability to respond appropriately during emergencies</li>\n</ul>\n<p><br></p>\n<p><span style=\"font-size: 18pt\">Why Join Our Team?</span></p>\n<p>This position offers the opportunity to make a direct and meaningful impact in someone’s daily life by helping them remain safely and comfortably in their own home while supporting the activities they enjoy most.
