    "description": "<p><span style=\"font-size: 24pt\">Registered Nurse (RN) – Hospice Home Visits</span></p>\n<p><span style=\"font-size: 18pt\">San Antonio, Texas | PRN or Part-Time</span></p>\n<p><a target=\"_new\" rel=\"noopener noreferrer\">Pacific Hospice and Palliative Care</a> is growing, and we are seeking a compassionate, experienced, and dependable Registered Nurse to join our dedicated hospice team serving patients throughout the San Antonio area.</p>\n<p>This position is ideal for an RN seeking a PRN, Part-Time, or On-Call opportunity providing in-home hospice care. The ideal candidate must have a minimum of two years of hospice experience, be bilingual in English and Spanish, and reside in the San Antonio area.</p>\n<p>At Pacific Hospice, we are committed to providing exceptional end-of-life care that honors dignity, comfort, and quality of life. We are looking for a nurse who combines strong clinical expertise with empathy, professionalism, and excellent communication skills.</p>\n<p><span style=\"font-size: 18pt\">Responsibilities</span></p>\n<ul>\n<li>Provide compassionate, patient-centered nursing care to hospice patients in their homes</li>\n<li>Perform comprehensive assessments, including pain and symptom management, psychosocial support, and ongoing evaluation of patient needs</li>\n<li>Develop and implement individualized care plans in collaboration with the interdisciplinary team, patients, and families</li>\n<li>Administer medications and treatments according to physician orders and hospice protocols</li>\n<li>Educate patients and caregivers regarding disease progression, symptom management, medications, and end-of-life care</li>\n<li>Provide emotional support and guidance to patients and families throughout the hospice journey</li>\n<li>Collaborate closely with physicians, social workers, chaplains, aides, and other team members to ensure continuity of care</li>\n<li>Maintain accurate and timely clinical documentation in compliance with hospice regulations and agency standards</li>\n<li>Participate in bereavement support and family follow-up as appropriate</li>\n<li>Stay current with hospice best practices through ongoing education and professional development</li>\n</ul>\n<p><span style=\"font-size: 18pt\">Qualifications</span></p>\n<ul>\n<li>Current and unrestricted Texas RN license</li>\n<li>Minimum of 2 years of hospice nursing experience required</li>\n<li>Strong clinical assessment and case management skills</li>\n<li>Experience with pain management, symptom control, and end-of-life care</li>\n<li>Bilingual English/Spanish preferred</li>\n<li>Proficient with electronic documentation systems, including Axxess or other hospice EMR platforms</li>\n<li>Comfortable using email, Slack, and other communication platforms</li>\n<li>Excellent verbal and written communication skills</li>\n<li>Compassionate, dependable, and team-oriented professional</li>\n<li>Knowledge of Medicare Conditions of Participation and Joint Commission standards preferred</li>\n<li>Reliable transportation and valid driver’s license required</li>\n</ul>\n<p><span style=\"font-size: 18pt\">Why Join Pacific Hospice?</span></p>\n<p>At Pacific Hospice and Palliative Care, you will be supported by a knowledgeable, compassionate, and collaborative team that values both clinical excellence and work-life balance.</p>\n<p>We offer:</p>\n<ul>\n<li>Competitive compensation</li>\n<li>On-Call opportunities</li>\n<li>Supportive leadership and team environment</li>\n<li>Meaningful work that makes a difference in the lives of patients and families every day</li>\n</ul>\n<p>Join the Pacific Hospice team and help provide comfort, dignity, and compassionate care when it matters most.</p>",
